Ocular side effects of systemic isotretinoin used for treatment of Acne vulgaris

Abstract

Background and design: Isotretinoin decreases sebum production in skin and various side effects most of which are dose-dependent and reversible may be observed. The aim of this study was to determine ocular adverse effects. Materials and methods: Fifteen patients with acne vulgaris who have been started on 0.5 mg/kg/day isotretinoin treatment were followed for ocular side effects for 4 months. Visual acuity measurement, Schirmer 1 test, floressein break up time (BUT), routine biomicroscopic and fundus examinations were performed and subjective complaints were inquired before treatment and on the first and fourth months of treatment. Results: All patients were normal before treatment with respect to ophtalmological findings. Conjunctival hyperemia was detected by biomicroscopic examination in 53.3% of patients after the first month of therapy; all of these patients had mild subjective dry eye complaints. While 'Schirmer 1 test was normal on the first month, it was below 10 mm in 20% of cases on the fourth month' (p=0.0025). BUT was below 10 seconds in 13.3% and 43.3% of patients on the first and fourth months of treatment respectively (p=0.0000). No detectable findings of cornea, lens and fundus occured. Conclusion: Systemic isotretinoin treatment can cause dry eye syndrome by affecting meibomian gland structure and functions causing destabilisation of the tears film and shortening of BUT. Dermatologists should be in cooperation with ophtalmologists.

Amaç: Temel etkisini yağ bezlerinin aktivitesini azaltarak gösteren isotretinoinin çoğu doza ve süreye bağımlı olan yan etkileri vardır. bu çalışmada olası oküler yan etkilerin saptanması amaçlanmıştır. Gereç ve Yöntem: 0,5 mg/kg/gün isotretinoin tedavisi başlanan 15 akne vulgarisli olgu 4 ay boyunca oküler yan etkiler açısından izlendi. Tedavi öncesi ve tedavinin 1. ve 4. aylarında görme keskinliği ölçümü, Schirmer 1 testi, floressein parçalanma zamanı (FPZ), rutin biyomikroskobik muayene ve göz dibi muayeneleri yapılıp subjektif yakınmalar sorgulandı. Bulgular: Tedavi öncesinde tüm olgular oftalmolojik açıdan doğal bulundu. Olguların biyomikroskobik muayenelerinde 1. aydan itibaren %53.3 oranda konjonktivada yüzeyel hiperemi izlendi; bu olguların tümü hafif derecede subjektif kuru göz semptomları tarif etmekteydi. Schirmer 1 testi 1. ayda tüm olgularda normalken 4. ayda %20 olguda 10 mm altında idi (p=0,0000). Olguların kornea, lens ve göz dibinde tespit edilebilen bir değişiklik olmadı. Gece körlüğü ve görme keskinliğinde azalma hiçbir olguda gelişmedi. Sonuç: Sistemik isotretionin tedavisi meibomian bez yapı ve fonksiyonlarını etkileyerek gözyaşı film tabakasının destabilazyonuna, FPZ' nin kısalmasına ve hafif derecede kuru göz sendromuna yol açabilmektedir. Bu tedaviyi uygulayan dermatologların göz hekimiyle kooperasyonu gerekmektedir.
