Grading of acne vulgaris: Different observers but same results?

Abstract

Background and design: Grading of acne vulgaris is problematic due to the polymorphic nature of the disease, difficulty in numeric evaluation of all parameters of the disease and interobserver variability. In this study two different methods of acne grading were compared and correlations of different methods and different observers were investigated. Materials and methods:69 acne vulgaris patients were independently evaluated by three dermatologists with a general acne score between 1 and 4. Then, a differential lesion count of all cases was performed by the same dermatologist. Relationship between the average general acne score and the scores of different observers was evaluated using Pearson's correlation coefficient Relationship between lesion counts and general scores was investigated by multiple variance analysis. Results: There was statistically significant correlation among different observers' scores (Pearson's coefficient: 0.88). Analysis of different lesions and acne score showed that the number of papulopustular lesions had the highest correlation with acne severity (%55). Conclusion: An overall acne severity score given after a routine dermatologic examination and with special consideration to papupopustular lesions is a sufficient measure for evaluating acne patients.

Bu çalışmada akne vulgarisli olguların dermatozlarının şiddetini değerlendirmede mevcut lezyonların sayımı ile genel derecelendirmeleri arasındaki ilişki incelenmiştir. 69 akne olgusu üç dermatolog tarafından genel akne şiddet skoru ile derecelendirilmiş ve tüm olguların aynı dermatolog tarafından lezyon sayımı yapılmıştır. Farklı gözlemcilerin akne şiddet skorlarının birbirleri ve her hasta için hesaplanan ortalama skor ilişkisi anlamlı bir korelasyon göstermiştir. Lezyon sayımı ile elde edilen verilerin ortalama genel akne şiddeti skoru ile ilişkisi araştırıldığında, en belirgin korelasyon papülopüstüler lezyonların dikkatlice gözden geçirilmesi sonucu verilecek genel bir skorun uygulamasının kolay, tekrarlanabilir ve en önemlisi de yeterli bir derecelendirme seçeneği olduğu kanısına varılmıştır.
