The incidence of genital herpes in patients with genital ulcers

Abstract
Background and design: The incidence of genital herpes is increasing worldwide. In recent studies herpes simplex virus type 2 (HSV-2) has been shown to be the most common cause of genital ulcers seen among the sexually transmitted diseases (STDS). This study was aimed to investigate the incidence of genital ulcers caused by HSV-2 in female patient population who were not in sexually transmitted risk group diseases. Materials and methods: We included 50 female patients with first episodes of genital ulceration who were admitted to dermatology and gynecology sections of Haseki Hospital between May 1999 and October 1999. The blood samples of all the patients were studied for VDRL, TPHA, anti HIV, HSV-2 IgG-IgM. All the patients were evaluated for the presence of Behçet's disease and were treated as per diagnosis. Results: In ten out of 50 patients with genital ulcers Behçet's disease (%20), in 5 patients active genital herpes (%10), in 1 patient Behçet's disease accompanying genital herpes (%2), in one patient syphilis (%2), in one patient squamous cell carcinoma (%2), and in one patient furuncle caused ulcer (%2) was found. In 17 out of 50 patients of genital ulcers HSV-2 IgG was pozitive (%34) and HSV-2 IgM was negative. Conclusion: Among the causes of genital ulcers in sexually transmitted disease patients the incidence of herpes genitalis was found to be higher than that of syphilis. The high rate of positive HSV-2 IgG (%34) was also thought to support the high incidence rate of herpes genitalis in our country.

Tüm dünyada genital herpes sıklığı giderek artmaktadır. Son yayınlarda cinsel ilişki ile bulaşan hastalıklara bağlı genital ülserlerin çoğunun herpes simpleks virüs Tip 2 (HSV-2) ye bağlı olduğu bildirilmektedir. Cinsel ilişkiyle bulaşan hastalıklar açısından risk grubunda olmayan kadın hasta grubunda HSV-2' ye bağlı genital ülser sıklığını araştırmak amacıyla yapılan bu çalışmaya Mayıs-1999-Ekim 1999 tarihleri arasında Haseki Hastanesi Dermatoloji ve Jinekoloji polikliniklerine genital ülser nedeniyle ilk kez başvuran 50 kadın hasta alınmıştır. Hastaların hepsinin serumlarında VDRL, TPHA, anti HIV, HSG-2 IgG ve IgM antikor testleri yapılmıştır. Tüm hastalarda Behçet hastalığı sorgulanmış ve araştırılmıştır. 50 genital ülserli hastanın 10' unda Behçet hastalığı (%20), 5' inde aktif genital herpes (%10), 1' inde Behçet hastalığı ile birlikte genital herpes (%2), 1'inde skuamöz hücreli karsinom (%2), 1'inde fronküle bağlı ülser (%2) saptanmıştır. 50 kişilik genital ülserli hasta grubumuzun 17'sinde (%34), HSV-2 IgM (-) bulunmuştur. Çalışma grubumuzda cinsel ilişki ile bulaşan hastalıklara bağlı ülser sebeplerinden herpes genitalisin sifilizden daha fazla görülmesi ve HSV-2 IgG (+) liğinin cinsel ilişki ile bulaşan hastalıklar açısından risk grubunda olmayan kadın hasta grubunda %34 gibi yüksek oranda bulunması sonucu genital herpesin ülkemizde de sık rastlanan bir hastalık olduğu kanısına varılmıştır.
