Evaluation of bacterial colonization in patients withthediagnosisof mycosis fungoides

Abstract

Background and Design: Mycosis fungoides (MF) is the most common form of cutaneous T-cell lymphoma (CTCL). Bacterial infections are a common complication of MF/Sezary syndrome (SS). Staphylococcus aureus is the most common causes of skin infections in patients with MF/SS. However, the role of infectious agents and their effects on the clinical course is controversial. In this study, we aimed to assess the prevalence of the colonization of bacterial pathogens among individuals with MF/SS in comparison with control subjects. Materials and Methods: Sixty-six patients with MF/SS and 66 healthy control subjects were included in this study. The subjects were swabbed in the nose, throat and axilla and the samples were evaluated according to the Clinical Laboratory Standards Institute (CLSI) criteria. Results: The mean age of the patients was 55.9±14.8 years (range: 21-84 years). Forty-eight patients were with ≤stage 2A, 18 were ≥stage 2B SS and MF. No statistically significant difference was observed in the frequency of pathogenic bacteria colonization in the nose and axilla between the two groups. However, in the throat it was statistically higher in MF/SS (p=0.001). There was no statistically significant difference between the two groups in terms of S.aureus colonization. The frequency of pathogenic bacteria colonization in the throat cultures were significant (p=0.004) for patients with late stage disease compared to those with early stages, but this was not shown in axilla and nose cultures. Conclusion: In this study, bacterial colonization in the throat and nasal carriage of S.aureus were found to be higher in patients with MF/SS. Further studies on the benefits of controlling this problem, possibly with simple and inexpensive methods that might ameliorates the symptoms such as itching, scaling and erythema are needed. Amaç: Mikozis fungoides (MF) derinin en sık görülen kutanöz T hücreli lenfomasıdır (KTHL). İnfeksiyon oluşumu MFli hastalarda en sık komplikasyonlardandır. MF/Sezary sendromlu (SS) hastalarda deri infeksiyonlarının en yaygın patojeni Staphylococcus aureusdur (S.aureus). İnfeksiyon ajanlarının, MF/SSnin oluşumundaki rolü ve hastalığın klinik seyrine etkileri ise tartışmalıdır. Bu çalışmada MF/SSli hastalarda, burun, aksilla ve boğazda patojen bakteriyel ajan kolonizasyonunun kontrol grubu ile karşılaştırıldığında yaygınlığının değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Çalışmaya 66 MF/SSli hasta ve kontrol grubunu oluşturmak üzere 66 sağlıklı kişi dahil edildi. Hastalardan burun, boğaz ve aksilla kültürleri alındı ve Clinical and Laboratory Standards Institute (CLSI) standartlarına göre değerlendirildi. Bulgular: Çalışmaya dahil edilen 66 MF/SSli hastaların yaş ortalaması 55,96±14,82 yıldı (21-84). MFli hastaların 48i ≤evre 2A, 18i ≥evre 2B ve Sezary sendromuydu (SS). MF/SSli hastalarda burunda ve aksillada patojen bakteri kolonizasyon sıklığı kontrol grubu ile karşılaştırıldığında istatistiksel olarak anlamlı fark saptanmazken, boğazda patojen bakteri kolonizasyonu MF/SS lehine istatistiksel olarak anlamlı fark oluşturdu (p=0,001). S.aureus kolonizasyon sıklığı açısından iki grup arasında istatistiksel olarak anlamlı fark saptanmadı (p=0,234). Erken evre MFli hastalar, geç evre MF/SSli hastalar ile boğazda patojen bakteri kolonizasyon sıklığı açısından karşılaştırıldığında geç evre lehine istatistiksel olarak anlamlı fark saptanırken (p=0,004), aksilla ve burun kültürlerinde fark saptanmadı. Sonuç: Bu çalışmada MF/SSli hastalarda boğazda bakteri kolonizasyonu ve burunda S.aureus taşıyıcılığı yüksek bulundu. Bu problemin basit ve maliyet etkin yöntemlerle kontrol altına alınması ile kaşıntı, kızarıklık ve skuamasyon gibi semptomlar üzerine düzeltici etkilerinin olup olmayacağı konusunda çalışmalara ihtiyaç duyulmaktadır.
