Is folic acid supplementation necessary for patients treated with methotrexate?

Abstract
Objective: The present open, controlled and randomized study aims to determine whether adding folic acid in the methotrexate treatment would reduce the side effects of methotrexate and its effect on psoriasis. Methods: Forty-two patients with psoriasis vulgaris were included in the study. All patients received 15-22.5 mg/week/po methotrexate, every 12 hours in 3 divided doses. Folic acid was given as 5 mg/day for the remaining 5 days of the week to the patients randomly allocated in the FA group. All patients were examined for clinical response and side effects, every 2 weeks. Clinical efficacy was evaluated by PASI. Results: No significant difference was found between the folic acid group and the group given no folic acid in terms of methotrexate efficacy (p=0.886 at baseline, p=0.668 at 8 weeks). Amaç: Bu çalışma, metotreksat tedavisine folik asit eklenmesinin, metotreksata bağlı gelişebilen yan etkilerin sıklığını ve metotreksatın psoriazisteki etkinliğini azaltıp azaltmayacağını değerlendirmek amacıyla yapılmıştır. Yöntem: Açık, kontrollü ve rastgele olarak planlanan çalışmaya, sistemik metotreksat tedavisine uygun, psoriazis vulgaris tanılı 42 hasta alındı. Tüm hastalara, 12 saat arayla 3 bölünmüş dozda toplam 15-22,5 mg/hafta oral metotreksat verildi. Rastgele olarak folik asit grubuna alınan hastalara tedavi dışındaki 5 günde 5 mg/gün folik asit desteği verildi. Hastalar 2 haftalık aralarla ayrıntılı olarak yan etki ve klinik yanıt açısından kontrol edildi. Klinik etkinliğin değerlendirmesi PASI ile yapıldı. Bulgular: Folik asit alan ve almayan hastaların tedaviye verdikleri yanıtlar arasında istatistiksel olarak anlamlı bir farklılık saptanmadı (Başlangıçta p=0.886, 8. haftada p=0.668). Çalışmayı tamamlayan 33 hastanın 26’sında (%79) metotreksat kullanımına bağlı yan etki gelişti.
