Abstract

Chronic actinic dermatitis (CAD) is characterized by persistent eczema-like lesions, mainly on sun-exposed sites, induced by ultraviolet B, sometimes ultraviolet A, and occasionally visible light. CAD is a rare photodermatitis. It is often associated with contact allergens including airborne allergens such as fragrances, plant antigens and topical medications. A 62 year old farmer is applied with eczematous lesions restricted to sun-exposed areas. Clinical findings and histopathologic features were consistent with the diagnosis of chronic actinic dermatitis. The patient also had contact allergy to multiple allergens. We present this case to emphasize the significance of patch test on CAD treatment and the success of topical tacrolimus and azathioprine.

Özet

Kronik aktinik dermatit (KAD) sıklıkla güneş gören bölgelerde ultraviyole B, bazen ultraviyole A ve nadiren de görünür ışıkla ortaya çıkan, inatçı egzama benzeri lezyonlar ile karakterizedir. KAD nadir görülen bir fotodermatozdur. Sıklıkla kokular, bitki antijenleri ve topikal ilaçlar gibi airborn allerjenleri içeren kontakt allerjenler ile ilişkilidir. Altmış iki yaşında çiftçilik ile uğraşan hasta güneş gören bölgelerine sınırlı egzamatöz lezyonlar ile başvurdu. Klinik ve histopatolojik bulgular KAD ile uyumlu olarak bulundu. Hastanın ayrıca pek çok allerjene karşı kontak allerjisi tespit edildi. Olgumuzu KAD tedavisinde patch testin önemini ve tedavide topikal takrolimus ile azatioprinin başarılı seçenekler olduğunu vurgulamak amacıyla sunuyoruz.
