HBV DNA detection in chronic hepatitis B infected patients by quantitative Hybrid Capture method and its clinical importance

Abstract

HBV is one of the causative agents of viral hepatitis. It is an important public health problem worldwide by causing acute and chronic infections. HBV DNA, is an important marker additional to the other serologic markers and gives idea about viral replication. While PCR technique detects HBV DNA qualitatively, Hybrid Capture tecnique detects it quantitatively. In this study, our aim was to compare PCR technique with hybridization method which gave quantitative results and the correlation of HBV DNA levels with ALT and AST levels. In 29 of 57 patients (%51) whom all were positive by PCR technique HBV DNA levels were found > 5pg/ml by quantitative Hybrid Capture technique. In 28 patients (%49) HBV DNA was found negative by quantitative Hybrid Capture technique. HBV DNA was not detected by quantitative Hybrid Capture technique in patients whose HBV DNA was found negative by PCR technique. A weak correlation was found between the serum levels of quantitative Hybrid Capture HBV DNA and serum ALT and AST levels (r=0,22, p-0,12; r=0,24, p=0,07 respectively). In conclusion, in patients with positive HBV DNA by PCR, the analytical sensitivity of quantitative Hybrid Capture HBV DNA technique was lower than that of PCR according to the results of detection of HBV DNA by quantitative Hybrid Capture technique. But quantitative Hybrid Capture technique was an appropriate way of detecting HBV DNA quantitatively. It was concluded that there is a statistically weak correlation between quantitative HBV DNA levels and serum ALT, AST levels. With these characterictics quantitative Hybrid Capture HBV DNA method is useful for the selection of antiviral therapy and f or the monitoring of the response to therapy in patients with chronic B hepatitis.

Özet

Hepatit- B Virüsü (HBV), viral hepatite neden olan ajanlardan biridir. Akut ve kronik enfeksiyon tablosu oluşturması nedeniyle bütün, dünyada önemli bir halk sağlığı problemidir. HBV DNA HBV enfeksiyonu tanısında diğer serolojik markırlara ilaveten,saptanan önemli bir belirleyicidir ve viral replikasyon hakkında bilgi vermektedir. PCR (polimeraz zincir reaksiyonu) HBV.DNA'yı kalitatif olarak saptarken, hibridizasyonu temel alan Hybrid Capture yöntemi ise HBV DNA'yi kantitatif olarak saptar. Bu çalışmada kantitatif sonuç veren hibridizasyon yönteminin PCR tekniği ile uyumunu ve aynı zamanda HBV DNA düzeyleri ile ALT (alanın aminotransferaz) ve AST (aspartat aminotransferaz) düzeyleri arasındaki ilişkinin saptanmasını amaçladık. PCR tekniği ile HBV DNA 'sı pozitif bulunan 57 hastanın 29'unda (%51) kantitatif Hybrid Capture yöntemiyle HBV DNA>5 pg/ml bulundu. Hastaların 28'inde (%49) ise kantitatif Hybrid Capture yöntemiyle HBV DNA negatif bulundu. HBV DNA'sı PCR tekniği ile negatif bulunan hastaların hiçbirisinde kantitatif Hybrid Capture yöntemiyle HBV DNA belirlenemedi. Kantitatif Hybrid Capture HBV DNA düzeyleriyle serum ALT ve AST düzeyleri arasında zayıf bir ilişki saptandı (Sırasıyla r=0,22, p=0,12; r=0,24, p=0,07). Sonuç olarak; PCR tekniği ile HBV DNA'sı pozitif bulunan hastalarda, kantitatif Hybrid Capture yöntemiyle yaptığımız HBV DNA tayininde elde ettiğimiz bulgularla kantitatif Hybrid Capture HBV DNA yönteminin analitik duyarlılığının PCR tekniğine göre daha düşük olduğu ancak HBV DNA 'yi kantitatif olarak belirlemede uygun bir yöntem olduğu ve kantitatif HBV DNA düzeyleri ile serum ALT ve AST düzeyleri arasında istatistiksel olarak zayıf bir ilişki bulunduğu sonucuna varıldı. Kantitatif Hybrid Capture HBV DNA tayin yöntemi, bu özellikleri ile kronik hepatit B enfeksiyonlu hastalarda antiviral tedavinin seçimi ve verilen tedavinin sonuçlarını gözleme açısından yararlı olabilir.
