Atypical cells in sysmex UN automated urine particle analyzer: a case report and pitfalls for future studies

Abstract
bjectives: Sysmex UN series fully automated urineanalyzer reports a series of research parameters besidesroutine parameters including the “atypical cells”parameter. An automated instrument in clinical usecapable of detecting neoplastic cells of the urinary tractwill have paramount significance.Case presentation: A 73 years old male patient with arecurrent high grade urothelial carcinoma admitted to oururology outpatient clinic due to hematuria.Urinalysis showed +3 hemoglobin, +3 leukocyteesterase, 200/HPF red blood cells, 300/HPF white blood cells.The instrument also reported atypical cells (7.6/µL or 1.3/HPF) under the heading of “research parameters.” Presenceof atypical cells was confirmed by the manual microscopy.The patient has undergone transurethral resection of papillary lesions and the pathology report confirmed a recurrence.On follow-up, atypical cells fell to 0.1/µL after 40 days.Conclusions: This case report presents a patient with atypicalcells in urine, detected by a fully automated urine analyzer.The atypical cells presented on the screen of the analyzerwere confirmed by the manual microscopy. This presentation may influence future studies pertaining to the subject.
Özet
Amaç: Sysmex UN Serisi tam otomatik idrar analizi cihazları rutin parametreler yanısıra araştırma parametreleri başlığı altında da bir dizi sonuç rapor etmektedir. Bunlardan biri de “atipik hücreler” parametresidir. Klinik kullanımdaki tam otomatik bir cihazın idrar yollarının neoplastik gelişimlerini tespit edebilmesinin çok yüksek yararlanımları olacaktır. Olgu: Reküren yüksek dereceli urotelyal neoplazi tanısı ile takip edilen 73 yaşındaki erkek hasta üroloji polikliniğine hematüri nedeniyle başvurmuştur. Sonuçlar: İdrar analizi +3 hemoglobin, +3 lökosit esteraz, 200/BBA kırmızı kan hücreleri, 300/BBA beyaz kan hücreleri ve 7.6/µL veya 1.3/BBA atipik hücreler gös termiştir. Atipik hücrelerin varlığı manuel mikroskopi ile de onaylanmıştır. Hastaya transuretral tumor cerrahisi uygulandı ve çıkarılan papiller lezyonların patolojik tanısı rekürrens olarak rapor edildi. 40 gün sonraki takipte idrar tahlilinde atipik hücrelerin sayısı 0.1/µL’ye düştü. Tartışma: Tam otomatik bir analizörde çalışılan idrar numunesinde atipik hücre tespit edilen bir olgu sunul maktadır. Cihazın tespit ettiği atipik hücreler manuel mik roskopi ile de görülmüştür. Bu sunum konu ile ilgili ilerki çalışmalar için ilham verici olacaktır.
