Abstract

This study assessed the effect of placebo ondansetron, tropisetron, granisetron and droperidol for prevention of PONV after gyneocological laparoscopy in patients receiving TIVA with remifentanyl and propofol. In this placebo-controlled, double-blind study included 75 female patients scheduled to undergo gynecological laparoscopic surgery. Anesthesia was induced with propofol 1-1.5 mg kg-1 IV and remifentanyl 1 µg kg-1 and was maintained with propofol 4 mg kg-1 hr-1 and remifentanyl 0.2 µg kg-1 min-1. Neuromuscular block was achieved with atracurium. Patients were randomized to receive placebo (Group I), ondansetron 4 mg (Group II), tropisetron 5 mg (Group III), granisetron 3 mg (Group IV), and droperidol 2.5 mg (Group V) within 20 min anesthesia induction. Remifentanil-propofol ile TIVA uygulanan, jinekolojik laparaskopi girişimlerinde ameliyat sırasında verilen plasebo, ondansetron, tropisetron, granisetron ve droperidolün postoperatif dönemde oluşan bulantı ve kusma üzerine etkileri araştırılmıştır. Çift kör ve plasebo kontrollü yapılan çalışma, jinekolojik laparoskopi uygulanacak 75 olguyu içermiştir. Anestezi indüksiyonu propofol 1-1.5 mg kg-1 ve remifentanil 1 µg kg-1, idamesi propofol 4 mg kg-1 sa-1 ve remifentanil 0.2 µg kg-1 dk-1 ile sağlanmıştır. Atrakuryum ile kas gevşekliği sağlanmıştır. Olgular rasgele 5 gruba ayrılarak Grup I'deki 15 olguya plasebo (serum fizyolojik), Grup II'deki 15 olguya 4 mg ondansetron ve Grup III'deki 15 olguya 5 mg tropisetron, Grup IV'deki 15 olguya 3 mg granisetron ve Grup V'deki 15 olguya 2.5 mg droperidol IV olarak, cerrahi başladıktan 20 dk sonra verilmiştir. Postoperatif 24 saatlik dönemde bulantı ve kusma sıklığı, antiemetik ihtiyacı ve taburcu özellikleri araştırılmıştır. Postoperatif 5. dk, 15. dk, 30. dk, 60. dk'larda plasebo grubunda görülen bulantı diğer gruplara göre anlamlı derecede yüksek olarak saptanmıştır (p
