A comparison of the success rates for tracheal intubation using the intubating laryngeal mask airway with and without fiberoptic guidance in patients with cervical pathology

Abstract
The aim of this prospective randomized study was to compare success rates for tracheal intubation using the intubating laryngeal mask airway (ILMA) with and without fiberoptic guidance in patients with cervical pathology. After Ethics Committee approval, 80 patients scheduled for elective servical disc operations, aged 18-78 years, and ASA I-II were included. After preoxygenation and standardized anesthesia induction protocol, the ILMA was inserted. After each placement, fiberoptic view of the airway was graded. Following successful placement, vecuronium 0.1 mg kg-1 was given. After 2 min., patients were intubated using ILMA with either fiberoptic guidance (The ILMA-FOB group, n=40) or without fiberoptic guidance (The ILMA-BLIND group, n=40). Times for successful placement of the ILMA and tracheal intubation, number of attempts needed for successful ventilation and tracheal intubation, and complications associated with procedure were recorded. The succcess rates of the first attempt and overall for the ILMA placement were 90 % and 95 % for the ILMA-BLIND group and also 90 % and 100 % for the ILMA-FOB group. Additionally, the succcess rates of the first attempt and overall for tracheal intubation were 80 % and 95 % for the ILMA-BLIND group and also 100 % and100 % for the ILMA-FOB group. In 2 patients of the ILMA-BLIND group, ventilation and tracheal intubation using the ILMA had failed at the third attempt. In 4 patients of the ILMA-FOB group, the ILMA was placed successfully at the second attempt. Intubation times were longer for the ILMA-FOB group (p< 0.001). The incidence of sore throat was 10% for the ILMA-BLIND group but 0 % for the ILMA-FOB group.

Prospektif, randomize çalışmanın amacı servikal patolojili olgularda fiberoptik yardımlı ve yardımsız "intubating" laringeal maske (İLMA) aracılığı ile endotrakeal entübasyonda başarı oranlarının karşılaştırılmasıdır. Etik Komite izni alındıktan sonra, elektif servikal disk ameliyatı planlanan 18-78 yaşları arası, ASA I-II grubu 80 olgu çalışmaya alındı. Preoksijenasyon ve standart anestezi indüksiyon protokolünün uygulanmasından sonra İLMA yerleştirildi. Her İLMA yerleştirilmesinden sonra, havayolunun fiberoptik görüntüsü değerlendirildi. Başarılı İLMA yerleştirilmesinden sonra 0.1 mg kg-1 vekuronyum uygulandı. İki dakika sonra, olgular İLMA kullanılarak ya fiberoptik yardımlı (İLMA-FOB grup, n=40), ya da kör (İLMA-KÖR grup, n=40) olarak entübe edildi.
