The effect of preemptive ketamine on analgesia and analgesic consuption after tonsillectomy

Abstract

We aimed to observe the effects of ketamine in subanaesthetic doses when used in different stages of surgery. The ethical committee approved the study. Forty-five patients aged between 5-15 scheduled for elective tonsillectomy/adenotonsillectomy were divided into equal three groups. The preemptive group (Group P) received ketamine 0.5 mg kg-1 IV in 2 mL saline at tonsillectomy position, followed by a continuous IV infusion of ketamine 6 µg kg-1 min-1, and during haemostasis 2 mL saline was administered. In the ketamine group (Group K) 2 mL saline were given at tonsillectomy position, and saline infusion (10 mL h-1) continued until haemostasis, when 0.8 mg kg-1 ketamine was given in 2 mL saline. In the control group, 2 mL saline was given at tonsillectomy position and saline infusion (10 mL h-1) continued until haemostasis. At the haemostasis 2 mL saline was administered. When VPS is greater than 3; tramadol 1 mg kg-1 IV in the first 6 hours, and after 6 hours paracetamol 40 mg kg-1 orally was given. Cardiores-piratory system data, extubation time, the post anaesthesia care unit (PACU) receiving time, discharging time from PACU, tramadol and paracetamol requirement, total dose of paracetamol, and complications were recorded. Recovery from anaesthesia and discharging parameters were similar between the groups. Total dose of paracetamol in the preemptive group was lower than the other groups. Çalışmamızda subanestezik dozdaki ketamini, cerrahinin değişik zamanlarında uygulayarak preemptif etkisini araştırmayı amaçladık. Etik kurul onayı sonrası genel anestezi altında adenotonsillektomi/tonsillektomi ameliyatu geçiren, 5-15 yaş arası 45 olgu üç eşit gruba ayrıldı. Grup P (Grup Pre-emptif)'deki olgulara tonsillektomi pozisyonunda ketamin, 0.5 mg kg-1, serum fizyolojik (SF) ile 2 mL'ye tamamlanıp intravenöz (İV) olarak, sonra kanama kont-rolüne kadar 6 µg kg-1 dk-1 infüzyonla verildi. Kanama kontrolünde 2 mL SF İV uygulandı. Grup K (Grup Ketamin)'daki olgulara tonsillektomi pozisyonunda SF; 2 mL İV, devamında kanama kontrolüne kadar 10 mL sa-1 infüzyonla uygulandı. Kanama kontrolünde 0.8 mg kg-1 ketamin SF ile 2 mL'ye tamamlanıp İV verildi. Grup C (Grup Kontrol)'deki olgulara SF; tonsil-lektomi pozisyonunda 2 mL İV, kanama kontrolüne kadar 10 mL sa-1 infüzyonla, kanama kontrolünde 2 mL İV verildi. Postoperatif analjezi, verbal pain skala skoru (VPS)>3 ise ilk 6 saatte tramadol 1 mg kg-1 İV, 6. saatten sonra parasetamol 40 mg kg-1 oral yolla sağlandı. Olgularda kardiyorespiratuar sistem bulguları, uyanma parametreleri, VPS skoru, tramadol ve parasetamole ihtiyaç, toplam parasetamol doz sayısı ve komplikasyonlar araştırıldı. Postoperatif VPS; preemptif grupta, erken dönemde diğer iki gruptan, 6. saatte kontrol grubundan daha düşüktü (p
