Anaesthetic management in Joubert syndrome: Case report

Abstract
Joubert syndrome is an autosomal recessive disorder, characterized by the presence of cerebellar vermis defects varying from hypoplasia to agenesis, episodic periods of tachypnea and apnea of neonatal onset; severe psychomotor delay, jerky eye movements, retinal dystrophy, rhythmic tongue protrusion, ataxia and hypotonia. Cerebellar abnormalities together with vermian agenesis reflect to clinical aspect with respiratory impairment in neonatal period especially. In this respect, respiratory effects of drugs and control of respiration are important in the anesthetic management of patients with Joubert syndrome. In this case report we evaluated the details of repeated anesthetic management of a child with Joubert syndrome.
Özet
Joubert Sendromu, hipoplaziden ageneziye kadar değişen ağırlıkta serebellar vermis defekti, neonatal dönemde epizodik takipne ve apne nöbetleri, ağır psikomotor gerilik, sıçrayıcı göz hareketleri, retinal distrofi, ritmik dil protrüzyonu, ataksi ve hipotoni ile karakterize otozomal resesif bir bozukluktur. Joubert Sendromunda serebellar vermis agenezisiyle birlikte görülen beyin sapı anormallikleri, kliniğe özellikle yeni doğan döneminde görülen solunum bozuklukları olarak yansımaktadır. Bu nedenle, Joubert sendromlu hastaların anestezi yönetiminde kullanılan ilaçların solunumsal etkileri ve solunumun kontrolü önem arzetmektedir. Bu olgu sunumunda Joubert Sendromlu hastamıza tekrarlayan genel anestezi uygulamalarımız irdelenmiştir.
