Combined liver and kidney transplantation from a living related donor in primary hyperoxaluria (Case report)

Abstract
Primary hyperoxaluria type 1 is an autosomal recessively inherited disorder caused by a deficiency of the hepatic alanine-glyoxylate aminotransferase, which leads to renal failure and subsequently to oxalate accumulation in various extrarenal tissues. We aimed to report our anesthetic managements of the recipient undergoing combined liver and kidney transplantation and the living donor.The recipient was a 9-year-old boy. He was hemodialised for 50 days until the operation day. Induction of anaesthesia was provided by thiopental sodium, and alfentanyl, and maintained by isoflurane and alfentanyl. Atracurium was used as neuromuscular blocker. Following lateral nephrectomy and native hepatectomy, the liver and the right kidney were transplanted. Both allografts started to function immediately after the reperfusion. The intraoperative period was uneventful, and inotropic support was not needed. During the operation, two units of packed red blood cells were transfused. Patient was extubated on the first postoperative day, transported to the ward on the third postoperative day.The donor was the patient's 19-year-old sister. Following the placement of an epidural catheter, general anaesthesia was provided by thiopental sodium, fentanyl, isofluran and vecuronium. In addition, bupivacaine and fentanyl were infused epidurally. The intraoperative period was uneventful; there was no requirement for inotropic agents. Two units of autologous blood were transfused during the operation. The donor was extubated in the first postoperative hour, and transported to the ward in the first day.In conclusion, taking into consideration of the pathophysiology of the proper timing of the operations contributed to an uneventful perioperative period in both the anaesthetic care of the donor and the recipient.

Primer hiperoksalüri Tip 1, hepatik alanin:gliyoksalat aminotransferaz enzimindeki eksikliğe bağlı olarak gelişen, böbrek yetersizliğine ve ekstrarenal dokularda oksalat birikimine neden olan otozomal resesif geçişli bir hastalıktır. Son evre böbrek yetersizliği gelişmiş primer hiperoksalüri tip 1 olgusuna aynı canlı vericiden tek seansta gerçekleştirilen kombine karaciğer böbrek transplantasyonunda alıcı ve vericiye ait anestezi deneyimimizin aktarılması amaçlandı.9 yaşındaki erkek çocuğa transplantasyon öncesi 50 gün süreyle hemodiyaliz uygulandı. Genel anestezi indüksiyonunda tiyopental sodyum, alfentanil, idamesinde alfentanil ve isofluran kullanıldı, nöromuskuler blok atrakuryum ile sağlandı. Bilateral nefrektomi ve nativ hepatektominin ardından, önce karaciğer sonra böbrek transplantasyonu tamamlandı. Greftler süratle fonksiyonel hale geldi. Peroperatif dönem sorunsuz seyretti, inotropik destek gerekmedi. İki ünite eritrosit, 1 ünite taze donmuş plazma verildi. Postoperatif 1. gün ekstübe edilip, 3. gün servise çıkarıldı.Olgunun 19 yaşındaki ablası olan vericiye epidural kateter yerleştirilmesinin ardından, genel anestezi tiyopental sodyum, fentanil, izofluran ve vekuronyum ile oluşturuldu. Ayrıca epidural yolla bupivakain ve fentanil infüzyonu da uygulandı. Peroperatif dönemde herhangi bir sorun yaşanmadı, inotropik destek gereksinimi olmadı. Toplam 2 ünite otolog kan transfüzyonu uygulandı. Postoperatif 1. saat sonunda ekstübe edilen olgu, 1. gün servise gönderildi.Anestezi uygulamasında hastalığın fizyopatolojisinin dikkate alınmasının, her iki ameliyatın zamanlamalarının doğru olmasının gerek alıcıda, gerekse vericide sorunsuz bir perioperatif dönem yaşanmasına katkıda bulunduğu sonucuna varıldı.
