The anaesthetic management of a patient with sarcoidosis (Case report)

Abstract

Sarcoidosis is a systemic granulomatous disease affecting all tissues and organs in the body. The anaesthesiologist must be aware of the patient's general status and must search for evidence of pulmonary and cardiac abnormalities in the preoperative period. In addition, close hemodynamic and respiratory monitoring should be used during the perioperative period. These patients may present many potential problems for the anaesthesiologists.In this case report, we show the succesful anaesthetic management of a patient with sarcoidosis under spinal anaesthesia. A 68-year-old female with sarcoidosis was scheduled for a total abdominal hysterectomy. She had no other past medıcal hıstory except hypertensıon. Perioperative monitoring included ECG, noninvasive blood pressure and peripheral oxygen saturation. We choose spinal anaesthesia over general anaesthesia because of the decreased respiratory and cardiac changes. After intravenous administration of 10 mL kg-1 0.9 % NaCl solution, the patıent was placed ın the left lateral posıtıon, and 22.5 mg of 7.5 % ropivacaine was injected into subarachnoid space via a 25 gauge spinal needle. The hemodynamic and respiratory status was stable and no complications were noted during anaesthesia.In conclusion, we think that spinal anaesthesia is safer than general anaesthesia for patients with sarcoidosis.

Özet

Sarkoidoz granulomatoz bir hastalık olup, herhangi bir doku veya organ etkilenebilir. Preoperatif dönemde anesteziyolog hastanın genel durumunun farkında olmalı ve kardiyak ve solunumsal patolojileri araştırmalıdır. Bunlara ek olarak, ameliyat sırasında yakın hemodinamik ve solunumsal monitorizasyon gereklidir. Bu olgular anesteziyolog için potansiyel problem oluşturabilir.Olgu sunumuzda, sarkoidozlu bir hastadaki başarılı spinal anestezi deneyimimizi bildirmekteyiz. 68 yaşında sarkoidozlu kadın hasta total abdominal histerektomi nedeni ile opere edildi. Anamnezinde hipertansiyon öyküsü mevcut olan olgumuza genel anesteziye göre minimal solunumsal ve kardiyak değişiklikler oluşturması nedeni ile spinal anesteziyi tercih ettik. Perioperative monitorizasyon, EKG, noninvaziv kan basıncı ve periferik oksijen satürasyonu monitorizasyonunu içermekteydi. % 0.9 NaCl solüsyonu 10 mL kg-1 iv uygulandıktan sonra, sol yan pozisyonda 25 gauge spinal iğne ile 22.5 mg (3 mL) % 7.5 ropivakain subaraknoid aralığa enjekte edildi. Anestezi süresince hemodinami stabil seyretti ve hiçbir komplikasyon ile karşılaşılmadı.Sonuç olarak, Spinal anestezinin sarkoidozlu hastalarda güvenli bir yöntem olduğunu düşünmekteyiz.
