Systemic effects of hypotensive anaesthesia during total hip replacement

Abstract

Hypotensive anaesthesia is used to reduce blood loss, and the risks associated with blood transfusion, to provide better surgical conditions and shorter duration of operation. The purpose of this study was to investigate the systemic effects of hypotensive anaesthesia. Thirty-seven (female 23, male 14) ASA physical status I - II patients, aged between 22-76 were randomised into two groups. In the hypotensive group (n=18), following epidural anaesthesia, nitroglycerine infusion was started to provide a mean arterial pressure of 60-70 mmHg before the surgical incision and discontinued before the closure of the incision. In the control group (n=19) only epidural normotensive anaesthesia was applied. Routine monitoring consisted of electrocardiography, peripheral oxygen saturation, central venous pressure, urine output and invasive arterial blood pressure. Duration of operation, peroperative hemodynamic changes, clinical cardiological signs and cerebral cognitive functions, the amount of blood loss and transfusion requirements were all noted. And also hematological, renal and hepatic function parameters were evaluated by blood samples taken in the preoperative period and in the first postoperative day.The demographic data, preoperative parameters, laboratory parameters and the duration of the operation were similar between the two groups. Hipotansif anestezi yöntemi kanama miktarını azaltarak daha iyi cerrahi görüş sağlamak, operasyon süresini kısaltmak, kan transfüzyon ihtiyacını azaltarak olası transfüzyon reaksiyonlarını önlemek için uygulanabilir. Çalışmamızda hipotansif anestezinin sistemler üzerine etkilerinin araştırılması amaçlanmıştır. Primer total kalça protezi uygulanacak, ASA I - II grubuna dahil, yaşları 22-76 arasında, 23 kadın ve 14 erkek toplam 37 olgu rasgele iki gruba ayrıldı. Epidural anestezi sonrası hipotansif gruba (n=18) cerrahi insizyondan önce, ortalama arter basıncı 60-70 mmHg olacak şekilde nitrogliserin infüzyonu başlandı, insizyon kapatılmaya başlanırken durduruldu. Kontrol grubuna (n=19) ise sadece epidural normotansif anestezi uygulandı. Tüm olgulara elektrokardiogram, periferik oksijen satürasyonu, santral venöz basınç, idrar çıkışı, invaziv kan basıncı monitörizasyonu uygulandı. Olguların operasyon süresi, peroperatif hemodinamik değişimleri, kardiyak yönden klinik şikayetleri, serebral yönden oryantasyon ve hafıza durum muayenesi, preoperatif ve postoperatif 1. gün alınan kan örnekleri ile hematolojik, böbrek ve karaciğer fonksiyon parametreleri, operasyon sırasında kanama miktarı, transfüzyon gereksinimi olan hasta sayısı ve miktarı kaydedildi. Her iki grubun karakteristik özellikleri, preoperatif parametreleri ile laboratuvar değerleri, operasyon süreleri benzerdi. Kanama miktarı, transfüzyon miktarı ve gereksinimi olan olgu sayısı hipotansif grupta istatistiksel olarak az bulundu (p< 0.05). Sonuç olarak, etkin monitörizasyon ve laboratuvar takipleri yapılarak, total kalça protezi cerrahisinde kan kaybının azaltılması ve diğer potansiyel avantajları nedeniyle hipotansif anestezi yöntemi tercih edilebilir.
