The anesthetic management of a patient who had bleomycin therapy (case report)

Abstract
It has been reported that high oxygen concentration during anaesthesia can increase pulmonary toxicity in patients treated with bleomycine. We report the succesful spinal anaesthetic management of a patient who had been treated with bleomycin.A 30 year-old man with a history of mixed germ cell testicular carcinoma post resectıon received seven courses of bleomycin therapy. Following bleomycin therapy, local recurrence developed in his scrotum and the patient required a second operation. Spinal anaesthesia was preferred over general anesthesia and 500 mL Ringer Lactate was infused before anaesthesia. With the patient in lateral decubitis position, a 25 gauge spinal needle was inserted through the L3-4 intervertebral space. After correct placement of the spinal needle, 15 mg of 0.5 % hyperbaric bupivacaine was administered. Hemodynamic and respiratory parameters were recorded during the operation. Nasal-oxygen was not used during intra- and postoperative periods because of bleomycin induced pulmonary toxicity. No complication was seen during intra- and postoperative periods.In conclusion, we think that spinal anaesthesia is a safe and effective anaesthetic technique for patients treated with bleomycin.

Bleomisin ile tedavi edilen hastalarda anestezi veya solunum tedavisi sırasında yüksek konsantrasyonda oksijen uygulamasının pulmoner toksisiteyi tetikleyebileceği veya artırabileceği bildirilmektedir. Bu olgu sunumuzda daha önceden bleomisin ile tedavi edilen bir hastadaki başarılı spinal anestezi yönetimini bildirmekteyiz.Otuz yaşında, daha önce mixt germ hücreli testis tümörü nedeni ile opere edilen erkek hasta, yedi kür bleomisin tedavisini takiben skrotumda lokal rekürrens gelişmesi nedeni ile yeniden opere edildi. Ameliyatun tekrarında olguda bleomisine bağlı pulmoner oksijen toksisitesinden kaçınmak amacı ile genel anestezi yerine spinal anestezi tercih edildi.Rutin EKG ve non invaziv kan basıncı monitorizasyonunu takiben 500 mL Ringer Laktat solüsyonu anestezi öncesi infüze edildi. Hasta sol yan pozisyonda iken 25 gauge spinal iğne ile L3-4 intervertebral aralıktan girildi. Spinal iğnenin doğru yerleşiminden sonra 15mg % 0.5 hiperbarik bupivakain intraspinal olarak uygulandı. Hemodinamik ve solunumsal parametreler ameliyat süresince kaydedildi. Sensoriyal blok düzeyi T5-6 düzeyine kadar ulaşan olguda intraoperatif ve postoperatif periyodda hipotansiyon, bradikardi gibi bir komplikasyon ile karşılaşılmadı. Bleomisine bağlı muhtemel pulmoner toksisite nedeni ile intraoperatif ve postopertif dönemlerde nazal oksijen kullanılmadı.Sonuç olarak, bleomisin ile tedavi edilen hastalarda oksijene bağlı pulmoner toksisiteden kaçınmak amacı ile spinal anestezinin güvenli ve etkin olduğu düşüncesindeyiz.
