Influence of antiseptic-impregnated central venous catheters on catheter colonization and catheter-related bacteremia incidences in patients receiving TPN

Abstract
Aim: Our study was performed to evaluate the impact of chlorhexidine and silver sulfadiazine-impregnated central venous catheters (CSS-SVC) on the incidence of colonization and catheter-related bacteremia in critically ill patients receiving TPN.Materials and methods: Triple-lumen CSS-SVC to 13 patients receiving TPN and standard triple-lumen central venous catheter (Standard-SVC) to 17 patients were inserted. SVCs were removed when no longer needed or when clinical signs of sepsis were observed.Results: Duration of catheterization and use of TPN, the catheter colonization and catheter-related bacteremia rates were similar for two groups. The catheter colonization was determined in 6 patients (% 46.2) (33.5/1,000 catheter days) of group CSS-SVK and 5 patients (% 29.4) (29.8/1,000 catheter days) of group Standard-SVK. The catheter-related bacteremia was found in 2 patients (% 15.1) (11.2/1,000 catheter days) of group CSS-SVK and 1 patients (% 5.9) (5.9/1,000 catheter days) of group Standard-SVK.Conclusion: We thought that the use of antiseptic-impregnated catheters has no effect on the incidence of both catheter colonization and catheter-related bacteremia in this study group however it's been concluded that the results has to be investigated in larger study groups.

Amaç: Çalışmamız, klorheksidin ve gümüş sülfadiyazin kaplı santral venöz kateterlerin (KGS-SVK) TPN uygulanan kritik olgularda kolonizasyon ve kateterle ilişkili bakteremi oranı üzerine etkisini değerlendirmek amacıyla yapıldı.Gereç ve Yöntem: TPN alan 13 olguda üç lümenli KGS-SVK, 17 olguda ise standart üç lümenli santral venöz kateter (Standart- SVK) uygulandı. SVK'ler gereksinim kalmadığında veya sepsisin klinik bulguları gözlendiğinde çıkarıldı.Bulgular: Kateterizasyon ve TPN kullanım süreleri, kateter kolonizasyonu ve kateterle ilişkili bakteremi oranları iki grup için de benzer bulundu. Kateter kolonizasyonu, KGS-SVK grubunda 6 olguda (% 46.2) (33.5/1,000 kateter günü), Standart-SVK grubunda 5 olguda (% 29.4) (29.8/1,000 kateter günü) saptandı. Kateterle ilişkili bakteremi ise, KGS-SVK grubunda 2 olguda (% 15.1) (11.2/1,000 kateter günü), Standart-SVK grubunda 1 olguda (% 5.9) (5.9/1,000 kateter günü) gözlendi.Sonuç: Bu çalışma grubunda antiseptik kaplı kateter kullanımının kateter kolonizasyonu ve kateterle ilişkili bakteremi oranları üzerine etkisinin olmadığı görülmekle birlikte daha fazla olguda araştırma gerektiği sonucuna varılmıştır.
