Anaesthetic technique and postoperative nausea, vomiting and cognitive functions in outpatients undergoing gynecologic laparoscopic surgery

Abstract

Aim: The aim of the study is to compare the effects of desflurane on postoperative nausea-vomiting and cognitive functions with prophylactic ondansetron to propofol anesthesia in outpatients undergoing gynecologic laparoscopic surgery.Materials and Methods: Sixty patients, between the ages 18-50 were studied. Before induction of general anaesthesia, all patients performed a brief cognitive test. Patients were randomized to three groups; Desflurane (group D); Desflurane-Ondansetron (group DO): and Propofol (group P). With exception of anaesthesia maintenance and ondansetron administration, anaesthetic care was standardized for all groups. Group DO received 4 mg ondansetron at induction. At the end of the operation, the Aldrete score was performed and when the Aldrete score was $geq$9 the patients were transported to the PACU. Nausea vomiting and VAS scores were determined at the 15, 30, 60 and 120 minutes and 24. hours following admission to the PACU. Cognitive function was evaluated 30 and 60 minutes postoperatively.Results: There were no significant differences in the demographic data or Aldrete and VAS scores of patients between groups (p>0.05). The incidence of nausea at postoperative 15 and 30 minutes was significantly higher in group D (p0.05).Conclusion: We conclude that for outpatient gynecologic laparoscopic surgery desflurane maintenance with ondansetron antiemetic prophylaxis is an alternative to propofol anaesthetic maintanence.

Özet

Amaç: Çalışmamızın amacı, günübirlik jinekolojik laparoskopik operasyonlarda desfluranın postoperatif bulantı, kusma, derlenme ve kognitif fonksiyonlar üzerine etkilerinin profilaktik ondansetron kullanımı ve propofol ile karşılaştırılmasıdır.Gereç ve Yöntem: Çalışmaya 18-50 yaş arasında 60 hasta dahil edildi. İndüksiyon öncesi kognitif fonksiyon testi uygulanan hastalar rastgele 3 gruba ayrıldı. Grup D: Desfluran, Grup DO: Desfluran-Ondansetron, Grup P: Propofol. Gruplarda anestezi idamesi ve ondansetron hariç standart anestezi uygulandı. Grup D'de 2 mL serum fizyolojik, Grup DO'da; 4 mg ondansetron (2 mL) uygulandı. Postoperatif dönemde Aldrete derlenme testi Aldrete skoru $geq$9 oluncaya kadar değerlendirildi; bulantı, kusma ve VAS değerleri 15., 30., 60., 90., 120. dakikalarda ve 24. saatte, kognitif fonksiyonları ise, 30. ve 60. dakikalarda değerlendirildi.Bulgular: Demografik veriler, Aldrete skoru veya VAS değerleri açısından üç grup arasında istatistiksel farklılık saptanmadı (p>0.05). Postoperatif 15. ve 30. dakikalarda desfluran grubundaki hastalarda daha fazla bulantı olduğu görüldü (p0.05).Sonuç: Günübirlik jinekolojik laparoskopilerde desfluran anestezisi, ondansetron ile anti emetik profilaksisi yapılarak propofol anestezisine alternatif olabilir kanısına varıldı.
