The comparison of different drug combinations in intravenous regional anaesthesia

Abstract
Aim: The addition of adjuvant drugs to local anaesthetics may limit toxic reactions in intravenous regional anaesthesia (IVRA) with little effect on the efficacy of the method.The present study was performed to determine the effect of adding adjuvant drugs on postoperative analgesia duration.Materials and Methods: Forty ASA I or II adult patients undergoing minor hand surgery under IVRA were randomly assigned to four grups including ten patients. IVRA was performed with 40 mL 0.9 % NaCl including lidocaine 0.5 % ( Group I), diphenhydramine 40 mg with lidocaine 0.25 % (Group II), tramadol 75 mg with lidocaine 0.25 % (Group III), tenoxicam 20 mg with lidocaine 0.25 % (Group IV).Vital signs (heart rate, mean arterial blood pressure) were recorded for the first 30 minutes intraoperatively, then for the first 15 minutes postoperatively.The onset and complete recovery of sensory and motor blockade, intraoperative quality of anesthesia, the onset of tourniquet pain and postoperative pain and side effects were recorded.Results: The onset of sensory and motor blockade was faster in Group I (lidocaine 0.5 %) and the onset of postoperative pain, delayed in Group IV (lidocaine+tenoxicam), when compared with other groups. There were no significant differences among the groups with respect to the tourniquet pain time and intraoperative anesthesia quality. There were no significant differences in vital signs or any other measured variable. There was no significant difference in intraoperative anaesthesia quality and the onset of tourniquet pain among the groups. We observed delayed onset of postoperative pain in the tenoxicam+lidocaine group.Conclusion: We concluded that the combination of adjuvant drugs to local anesthetics may be useful in IVRA.

Amaç: Bu çalışma, intravenöz rejyonel anestezi (İVRA) uygulamalarında lokal anestezik ajanlar nedeniyle oluşabilecek toksik reaksiyonların, yöntemin etkinliği azaltılmadan önlenebilmesi için eklenen adjuvanların, postoperatif analjezi süresine etkisini araştırmak amacıyla gerçekleştirildi.Gereç ve Yöntem: Küçük el cerrahisi geçirecek, ASA I-II toplam 40 hasta 10'ar kişilik 4 gruba ayrıldı. I. gruba % 0.5 lidokain, II. gruba % 0.25 lidokain + 40 mg difenhidramin, III. gruba % 0.25 lidokain + 75 mg tramadol, IV. gruba % 0.25 lidokain + 20 mg tenoksikam 40'ar mL % 0.9 NaCl ile sulandırılarak İVRA uygulandı. Tüm olgularda vital bulgular (kalp atım hızı, ortalama, sistolik, diyastolik arter basınçları) intraoperatif 30, postoperatif 15 dk. boyunca izlendi. Duyu ve motor bloğun başlangıç ve geri dönüş süreleri, intraoperatif anestezi kalitesi, postoperatif ağrı ve turnike ağrısı başlama zamanları ve yan etkiler değerlendirilerek kaydedildi.Bulgular: Duyu ve motor blok % 0.5'lik lidokain uygulanan grup I'de daha erken gelişti. Postoperatif ağrı başlama zamanı grup IV'de (tenoksikam +lidokain), diğer gruplardan daha uzundu. Turnike ağrısı başlama zamanı, intraoperatif anestezi kalitesi açısından gruplar arasında fark saptanmadı.
