Comparison of anaesthetic efficiency and neurotoxicity of intrathecal magnesium sulfphate and lidocaine in rabbits

Abstract
Aim: In our study, we investigated the spinal anaesthetic property and neurotoxicity of magnesium sulphate (MgSO4)which is an NMDA receptor antagonist during intrathecal injection to rabbits.Materials and Methods: Twenty (New Zealand White Male) rabbits were randomized into 4 groups. In the first group 0.15 mL of lidocaine 2 %, in the second group 0.15 mL of lidocaine 5 %, in the third group 0.15 mL of MgSO4 15 % and in the last group 0.15 mL of NaCl 0.9 % were injected to subarachnoid space by mediating with a nerve stimulator. We evaluated the motor block levels using the four level scale and sensorial block levels using forceps-pinch method at 1., 2., 3., 4., 5., 15., 30., 45. and 60. minutes after administration of the drugs. Eight days after this procedure, the rabbits were sacrificed and laminectomy was performed for each. We obtained tissue samples from the cervical, thoracal, lumbar and sacral segments of the medulla spinalis. Then we examined the neurotoxicity changes histopathologically under the light microscope .Although no motor and sensorial blockage were seen in MgSO4 and NaCl groups, all the rabbits in lidocaine 2 % and 5 % groups had motor and sensorial blockage.Results: In MgSO4 and lidocaine % 5 groups, histopathological neurotoxicity changes were found more than the changes in lidocaine % 2 and NaCl groups.Histopathologically, neurotoxicity changes were more pronounced in MgSO4 and lidocaine 5 % groups and statistically significant difference was found when compared with the serum physiologic group.Conclusion: We think that more studies are needed to use MgSO4 as a central neural blocking agent although clinically no neurotoxicity was observed.

Amaç: Çalışmamızda doğal bir N-metil-D-aspartat (NMDA) reseptör antagonisti olan magnezyum sülfatın (MgSO4), tavşanlarda intratekal olarak verildiğinde spinal anestezi oluşturup oluşturmadığını ve MgSO4 ile lidokainin nörotoksik olup olmadığını incelemeyi amaçladık.Gereç ve Yöntem: Yirmi tavşan randomize olarak 4 gruba ayrıldı. Sinir stimülatörü aracılığı ile subaraknoid aralığa birinci gruba 0.15 mL % 2'lik lidokain, ikinci gruba 0.15 mL % 5'lik lidokain, üçüncü gruba 0.15 mL % 15'lik MgSO4 ve son gruba ise 0.15 mL % 0.9 serum fizyolojik verildi.Subaraknoid enjeksiyonu takiben 1., 2., 3., 4., 5., 15., 30., 45. ve 60. dakikalarda motor blok skorlaması yapıldı. Duyusal blok olup olmadığı forceps-pinch testiyle değerlendirildi.Enjeksiyondan 8 gün sonra deneklerin laminektomileri yapılarak servikal, torakal, lomber ve sakral seviyelerinden medulla spinalis doku örnekleri alındı ve ışık mikroskobik olarak incelendi. Histopatolojik incelemede nörotoksisite bulguları araştırıldı.Bulgular: MgSO4 ve serum fizyolojik verilen gruplarda motor ve duyusal blok görülmedi. Lidokain verilen gruplarda ise, deneklerin tümünde hem motor blok hem de duyusal blok belirlendi.Medulla spinalisin histopatolojik incelemesinde (servikal, torakal, lomber ve sakral), serum fizyolojik verilen grup ile karşılaştırıldığında intratekal MgSO4 ve % 5'lik lidokain kullanılan gruplarda daha fazla denekte nörotoksik bulguya rastlandı.Sonuç: İntratekal MgSO4'ın klinik olarak duyu bloğu oluşturmaması ve histopatolojik incelemede medulla spinaliste nörotoksisite bulgularının belirlenmesi, bu ajanın spinal anestezide lokal anesteziklere iyi bir alternatif olamayacağını düşündürmüştür.
