Is timing important for postoperative pain management after thyroidectomy operation

Abstract

Aim: The aim of this study is to evaluate the effects of lornoxicam given in different periods of the surgery and tramadol given preoperatively on postoperative pain management and tramadol consumption.Material and Method: 75 ASA I-II patients undergoing thyroidectomy operations were randomly allocated to 3 groups. Lornoxicam 8 mg and 4 mg was given to Group LP and Group LL respectively and tramadol 100 mg was given to Group T. At the end of the surgery, lornoxicam 4 mg was given to Group LL and 2 mL sodium chloride was given to Group LP and Group T. Patients were evaluated in terms of headache, neck and shoulder pain (at rest, during movement and coughing) and patient satisfaction scores. The first analgesic requirement, maximum pain score and the time of maximum pain score and total amount of tramadol requirement during the first twenty-four hours and side effects were recorded. Results: Demographic data was similar in all groups. Pain scores at rest and during movement was found to be higher in Group LL in the first 10th, 15th and 30th minutes. Neck pain scores during coughing was found to be higher in Group LL in the first 10th and 15th minutes. There was no signifcant difference between groups in terms of pain scores in the other measurement intervals. There was no significant difference in terms of patient satisfaction scores in the first sixteen hours, but patient satisfaction scores in the 20th and 24th hours was found to be lower in Group T. During the postoperative period, the number of patients experiencing nausea in Group T was higher than the other groups. Conclusion: Lornoxicam has the similiar efficacy with tramadol in postoperative pain management in patients undergoing tiroidectomy.

Giriş: Bu çalışmanın amacı operasyonun farklı evrelerinde uygulanan lornoksikam ile preoperatif tramadolun postoperatif dönemdeki ağrı tedavisindeki etkinliğinin ve tramadol kullanımı üzerine etkilerinin incelenmesidir. Gereç ve Yöntem: Tiroidektomi operasyonu geçirecek 75 ASA I-II hasta 3 gruba ayrıldı. Grup LP’ye ve Grup LL’ye sırasıyla 8 mg ve 4 mg lornoksikam ve Grup T’ye 100 mg tramadol uygulandı. Cerrahi bitiminde Grup LL’ye 4 mg lornoksikam, Grup LP ve Grup T’ye 2 mL serum fizyolojik uygulandı. Hastalar ekstübasyon sonrası baş ağrısı, boyun ağrısı ve omuz ağrısı (dinlenim, boyun hareketi ve öksürme esnasında) ve memnuniyet skoru açısından değerlendirildi. Postoperatif dönemde ilk analjezik uygulama zamanı, maksimum ağrı skoru, maksimum ağrı skoru zamanı ve cerrahi sonrası ilk 24 saatteki toplam tramadol kullanımı ve yan etkiler kaydedildi. Bulgular: Demografik özellikler açısından gruplar benzer bulundu. Gruplar arasında ağrı skorları, dinlenim ve hareket s›ras›nda Grup LL’de ilk 10, 15 ve 30. dakikalarda daha yüksek bulunmuştur. Öksürme ile boyun ağrı skorları Grup LL’de ilk 10 ve 15. dakikalarda daha yüksek bulunmuştur. Diğer ölçüm aralıklarında ağrı skorları açısından gruplar arasında fark bulunmamıştır. Hasta memnuniyeti skoru incelendiğinde, ilk 16 saatlik dönemde gruplar arasında fark bulunmazken, 20 ve 24. saat ölçümlerinde preoperatif tramadol uygulanan grupta hasta memnuniyeti skoru daha düşük bulunmuştur. Postoperatif dönemde bulantı gözlenen hasta sayısı Grup T’de diğer gruplardan daha yüksek bulunmuştur. Sonuç: Tiroid cerrahisi sonrası ağrı tedavisinde lornoksikam kullanımı postoperatif ağrı tedavisinde tramadol ile benzer etkinliğe sahiptir.
