Anaesthetic management of a child with mucopolysaccharidosis type II and congenital hypotyroidism

Abstract
Mucopolysaccharidosis Type II (Hunter’s syndrome) is a rare and progressive X linked recessively inherited lysosomal storage disease caused by absence or insufficiency of enzyme catalyzing the metabolism of mucopolysaccarides. Rarely Hunter’s syndrome accompanied with congenital hypotyroidism. Abnormal accumulation and deposition of mucopolysaccharides in the tissue of several organs leads to numerous anatomical, musculoskeletal and neurological abnormalities that are known to complicate anaesthetic and airway management. Airway obstruction in mucopolysaccharidosis patients is caused by a combination of narrowed trachea, thickened vocal folds, macroglossia, a short neck and mucoid secretions. Especially temporomandibular joint arthritis frequently make the intubation very difficult. In this case report, we aimed to report the anaesthetic management of a patient with Hunter’s syndrome and congenital hypotyroidism who had undergone adenotonsillectomy with bilateral middle ear drainage tubes within the scope of literature.
Özet
Mukopolisakkaridoz Tip II (Hunter Sendromu) X’e bağlı resesif geçiş gösteren ve mukopolisakkaridlerin metabolizmasından sorumlu enzimin yokluğu veya yetersizliği sonucu ortaya çıkan ender fakat ilerleyici bir lizozomal depo hastalığıdır. Bu hastalığa konjenital hipotiroidinin eşlik etmesi oldukça enderdir. Mukopolisakkaritlerin çeşitli organ dokularında birikmesi anatomik, kas-iskelet ve nörolojik sistemleri etkileyerek anestezik ve hava yolu ile ilgili girişimleri zorlaştırmaktadır. Daralmış trakea, vokal kort kalınlaşması, makroglosi, kısa boyun ve mukoid sekresyon hava yolu obstrüksiyonuna neden olabilmektedir. Özellikle de temporomandibuler eklem artriti entübasyonu zorlaştırabilmektedir. Bu nedenle Hunter sendromlu hastalarda zor entübasyon olasılığına karşı fiberoptik bronkoskopla entübasyon ve acil trakeostomi için hazırlıklı olunmalıdır. Bu olgu sunumunda Hunter sendromuna eşlik eden konjenital hipotiroidisi olan bir hastada adenotonsillektomi ve kulaklara tüp uygulaması nedeniyle verilen genel anestezi uygulamamızı literatür eşliğinde tartışarak sunmayı amaçladık.
