The effects of sevoflurane anesthesia in two different fresh gase flow on anesthesia cost, hepatic and renal functions

Abstract
Aim: The aim of this study is to investigate the effects of 6 L min-1 and 3 L min-1 fresh gase flow during the sevoflurane anesthesia on the anesthesia cost, renal and hepatic functions. Materials and Methods: This study was performed on 90 patients, aged between 18-50 years, scheduled for elective surgery. Semi-closed respiration system was used for anesthesia. Patients were randomized into two groups. In both groups for first 5 minutes, anesthesia was maintained with % 1-2 sevoflurane in 6 L min-1 (2 L min-1 O2 + 4 L min-1 N2O) of fresh gas flow and after then, anesthesia was maintained with % 1-2 sevoflurane concantration in 3 L min-1 of fresh gas flow (1 L min -1 O2 + 2 L min-1 N2O) in Group I ( n=45) and 6 L min-1 of fresh gas flow (2 L min-1 O2 + 4 L min-1 N2O) in group II ( n=45). Systolic arterial pressure (SAP), diastolic arterial pressure (DAP), heart rate (HR), end-tidal carbondioxide (ETCO2) and end-tidal sevoflurane (ETsevo) were recorded during the operation at 15 minute intervals. Arterial blood samples were taken for arterial blood gas analysis and venous blood samples were taken for blood urea nitrogen (BUN), creatinine, aspartate amino transferase (AST), alanine amino transferase (ALT), gama glutamyl transferase (GGT) and total bilirubine levels. Sevoflurane consumptions for all patients were recorded.Results: There were no statistical difference in HR, SAP, DAP, SpO2, EtCO2, Etsevo, SaO2, PaO2, PaCO2, pH and serum AST, ALT, GGT, total bilirubine, BUN and blood creatinine levels between the groups (p>0.05). Amaç: Volatil anesteziklerden sevofluran ile anestezi uygulanan olgularda, taze gaz akımının 6 L dk-1’dan 3 L dk-1’ya düşürülmesi ile maliyete ve bu akım oranlarında karaciğer ve böbrek fonksiyonlarına etkisinin araştırılması.Gereç ve Yöntem: Çalışma, orta süreli elektif cerrahi uygulanan, 18-50 yaş arası 90 olguda gerçekleştirildi. Anestezide yarı-kapalı solunum devresi kullanıldı. Olgular rasgele randomize edilerek 2 gruba ayrıldı. Olgulara ilk 5 dk.’da 6 L dk-1 taze gaz akımı (2 L dk-1 O2 + 4 L dk-1 N2O) içinde %1-2 konsantrasyonda sevofluran ile anestezi uygulamasına başlandıktan sonra, Grup I olgularda (n=45) 3 L dk-1 (1 L dk-1 O2 + 2 L dk-1 N2O); Grup II olgularda (n=45) 6 L dk-1 (2 L dk-1 O2 + 4 L dk-1 N2O) taze gaz akımı içinde %1-2 konsantrasyonda sevofluran inhalasyonu ile anestezi uygulaması sürdürüldü. Ameliyat sırasında her 15 dk.’da bir kalp atım hızı (KAH), sistolik kan basıncı (SKB), diyastolik kan basıncı (DKB), periferik oksijen satürasyonu (SpO2), end-tidal karbondioksit (ETCO2), end-tidal sevofluran (ETsevo) kaydedildi. Arteryel kan gazı için arteriyel kan ve kan üre azotu (BUN), kreatinin, aspartat amino transferaz (AST), alanin amino transferaz (ALT), gama glutamil transferaz (GGT) ve total bilirubin için ise venöz kan örnekleri alındı. Her olgunun sevofluran tüketimi hesaplandı ve kaydedildi. Bulgular: Olguların KAH, SKB, DKB, SpO2, EtCO2, ETSEVO, arteryel kan gazı SaO2, PO2, PCO2, pH ve serum AST, ALT, GGT, BUN, total bilirubin, kreatinin düzeylerinde gruplar arasındaki fark istatistiksel olarak anlamlı değildi (p>0.05).
