Comparison of acetaminophen and lornoxicam given peroperatively on postoperative pain management in septoplasty and FESS

Abstract

Aim: Septoplasty and functional endoscopic sinus surgery (FESS) are common procedure done by otolaryngologists. However postoperative pain management is still challenging in these cases. The aim of this study is to compare the effectiveness of iv acetaminophen and lornoxicam given peroperatively, on postoperative pain management in septoplasty and FESS cases.Material and Method: 62 ASA I-II patients undergoing septoplasty and FESS operations were randomly allocated to 2 groups. Lornoxicam 8 mg and iv acetaminophen 1 gr was given to Group L and Group P respectively. Patients were evaluated after extubation, in terms of pain at rest, VRS, PRS and patient satisfaction scores. The first analgesic requirement, maximum pain score and the time of maximum pain score and total amount of tramadol requirement during the first 24 hours and side effects were recorded. Results: Demographic data was similar in all groups. There was no signifcant difference between groups in terms of pain scores at rest in the all measurement intervals. During the first 24 hours there was no significant difference for nausea, vomiting and severe pain incidence. In group L 8 % of the patients and in group P 4 % of patients were unsatisfied respectively. There was no significant difference in terms of patient satisfaction scores in the first 24 hours. At 48th hour, during nasal packing removal there were no significant bleeding or oozing.Conclusion: Acetaminophen when used as a single agent, has the similiar efficacy with lornoxicam in postoperative pain management in patients undergoing septoplasty and FESS.

Giriş: Septoplasti ve fonksiyonel endoskopik sinüs cerrahisi (FESC) otolaringolojistler tarafından günümüzde sıkça uygulanan bir ameliyattır. Ancak, bu olgularda postoperatif ağrı tedavisi hala tartışmalıdır. Bu çalışmanın amacı intraoperatif dönemde uygulanan parenteral asetaminofen ile lornoksikamın, septoplasti ve FESC’nin beraber yapıldığı ameliyatlarda ağrı tedavisindeki etkinliklerinin karşılaştırılmasıdır. Gereç ve Yöntem: Septoplasti ve FESC geçirecek. 62 ASA I-II hasta 2 gruba ayrıldı. Anestezi indüksiyonunu takiben Grup L’ye ve Grup P’ye sırasıyla 8 mg lornoksikam ve 1 gr asetaminofen parenteral yoldan uygulandı. Hastalar ekstübasyon sonrası dinlenim sırasında ağrı, VRS, PRS ve hasta memnuniyeti açısından değerlendirildi. Postoperatif dönemde ilk analjezik uygulama zamanı, maksimum ağrı skoru, maksimum ağrı skoru zamanı ve cerrahi sonrası ilk 24 saatteki toplam tramadol kullanımı ve yan etkiler kaydedildi. Bulgular: Demografik özellikler açısından gruplar benzer bulundu. Ağrı skorları incelendiğinde, dinlenim ile gruplar arasında tüm takip zamanları içerisinde ağrı düzeyleri yönünden istatistiksel olarak anlamlı farklılık saptanmadı. İlk 24. saate bulantı, kusma ve şiddetli ağrı yakınmalarının görülme sıklığında bir farklılık görülmedi. Hasta memnuniyeti incelendiğinde, ilk 24 saatlik dönemde Grup L’deki hastaların % 8’i memnun olmadıklarını belirtti. Grup P’de hastaların % 4’ü memnun değil olarak değerlendirildi. Hasta memnuniyeti açısından anlamlı fark saptanmadı. Postoperatif 48. saatte tamponlar çekilirken hastalarda belirgin bir kanama veya kan sızıntısı tespit edilmedi. Sonuç: FESC sonrası ağrı tedavisinde i.v. asetaminofenin tek ajan olarak uygulanması postoperatif ağrı tedavisinde i.v. lornoksikam ile benzer etkinliğe sahiptir.
