The effects of peribulbar and topical anesthesia in cataract surgery on patient and surgeon comfort

Abstract

Aim: The purpose of this study was to compare the effects of peribulbar and topical anesthesia in cataract surgery on patient and surgeon comfort, anesthetic agent requirement, adverse medical events and the relationship of anesthetic method with these parameters.Material and Method: After the approval of Institutional Review Board 40 patients (ASA I-III, 45-85 years) undergoing cataract surgery with facoemulsification method were randomly divided into two groups. Twenty of them underwent cataract surgery under peribulbar anesthesia (group LA), and 20 under topical anesthesia (group TA). After 5 minutes from patient sedation venous blood was taken for blood-gas values. Pain of the patients was evaluated by Visual Analogous Scale (VAS) at the onset, 10 minute intervals intraoperatively, at the end, 1 and 24 hours after operation. Assessment of level of sedation was made by using part of The Observer’s Assessment of Alertness and Sedation (OAA/S) scale. Awaking time and need for additional anesthetic drug were recorded. At the end of the surgery; cooperation of the patients was evaluated by the anesthesiologist.Results: There were no differences in VAS scores intraoperatively and postoperatively including 1. and 24. hour scores, and also no difference was found about anesthetic requirement between two groups (p>0.05). Özet

Amaç: Yaptığımız bu çalışmada, sedasyonla yapılan topikal veya peribulbar anestezi ile gerçekleştirilen katarakt cerrahisinin hasta konforu, cerrah konforu, sedatif anestezik gereksinimi, olumsuz medikal olaylar ve bunların anestezi yöntemi ile ilişkilerinin karşılaştırılması amaçlanmıştır.Gereç ve Yöntem: Etik kurul onayı alınarak elektif katarakt cerrahisi planlanan (ASA I-III, 45-85 yaş) fakoemülsiyon yöntemi ile opere edilecek kırk hasta randomize olarak iki gruba ayrıldı. Yirmi hastaya topikal anestezi (Grup TA), diğer yirmi hastaya da peribulbar anestezi (Grup LA) uygulandı. Hastalar EKG, noninvazif kan basıncı ve pulse oksimetre ile monitorize edildikten sonra sedatize edilip 5 dk. sonra venöz kan gazı alındı. Hastaların ağrıları VAS ile ameliyat başladığında, intraoperatif 10 dk. aralıklarla, postoperatif birinci ve yirmi dördüncü saatte değerlendirildi. Uyanıklık seviyesini ölçmede Uyanıklık/Sedasyon değerlendirme skalasının (OAA/S) yanıt verebilirlik kısmı kullanıldı. Uyanma süresi ve intraoperatif ek anestezik gereksinimi kaydedildi. Ameliyat bitiminde hasta kooperasyonu cerrah tarafından değerlendirildi.Bulgular: Grup TA ve LA arasında intraoperatif dönemde, postoperatif birinci ve yirmi dördüncü sa’te VAS değerleri ve ek anestezik ilaç kullanımı açısından fark bulunmadı.
