Anesthesia management in ankylosing spondylitis

Abstract
Ankylosing spondylitis is defined as an inflammatory arthritis of the spine always involving the sacroiliac joints. Cervical column and atlantooccipital articulation mobility are reduced and in severe cases the cervical vertebrae become fixed in a flexed position. This portion of the spine is also the most susceptible to fracture, particularly in hyperextension, an event that could lead to damage to the cervical spinal cord during maneuvers to manage the airway. For the anesthesiologists, the establishment and maintainence of airway is the most important problem in the cases with ankylosing spondylitis. In this case report, we describe the airway management of a patient with ankylosing spondylitis who would be ureterorenoscopy under general anaesthesia and we review the anaesthesia literature related to this disease.
Özet
Ankilozan spondilit, granulasyon dokusunun ligamentlerin kemik birleşimine ve eklem kapsülüne infiltrasyo-nunun söz konusu olduğu inflamatuar artritdir. Şiddetli olgularda servikal vertebra ve atlantoksipital eklem tutulumuyla hareket kısıtlılığı gelişmektedir. Bu nedenle özellikle başın hiperekstansiyonunda servikal spinal korddda hasar oluşabilir. Anesteziyologlar için ankilosan spondilitli olgularda hava yolu yönetimi önemli sorundur. Bu olgu sunumunda, genel anestezi altında ureterorenoskopi planlanan bir olguya sorunsuz anestezik yaklaşım, literatür bilgileri gözden geçirilerek tartışılmıştır.
