Abstract

Multiple sclerosis (MS) is a chronic demyelinating disease of the central nervous system affecting primarily women at the age between 30-40 years. The disease may affect many organ systems and the insufficiency caused in these systems may affect the anaesthetic management. The main point about MS is that it may have different symptoms and the degree of the disease may be changing. Although, all anaesthetic techniques may cause exacerbation of the disease, general anaesthesia and epidural anaesthesia with low concentration of lidocain seems safer than spinal anaesthesia. Patient should be informed that the disease may exacerbate after the surgery and the anaesthesia. Any potential efficacy, risks and complications of the anaesthetic technique should be explained. Evaluating and recording preoperative and postoperative neurological and respiratory data of the patient is important in respect to determine the postoperative effects of the surgery and the anaesthesia. Pharmacological treatment of the patient should be analyzed and if necessary replacement therapy be ordered. Body temperature should be monitored closely and hyperthermia should be prevented. In this paper, etiologies, progress and the anaesthetic management of MS was discussed.

Özet

Multipl skleroz (MS) özellikle 30-40 yaşlarında kadınları etkileyen santral sinir sisteminin kronik demiyelinizan bir hastalığıdır. Hastalık, pek çok organ sistemini etkileyebilir ve bu sistemlerde yetmezlik oluşturması durumunda hastaya verilecek anesteziyi de etkiler. MS hakkında anımsanması gereken en önemli nokta, çok çeşitli semptomlarının olması ve semptomlarının ağırlığının değişik derecelerde görülebilmesidir. Bütün anestezik teknikler hastalık semptomlarının alevlenmesi ile ilişkili olsa da, genel anestezi ve düşük konsantrasyonda lokal anestezikle uygulanacak epidural anestezinin, spinal anesteziden daha güvenli olabileceği bildirilmektedir. Hastalar, MS semptomlarının cerrahi ve anestezi sonrası alevlenebileceği konusunda bilgilendirilmeli ve uygulanacak anestezi tekniğinin olası yararları, riskleri ve komplikasyonları anlatılmalıdır. Preoperatif ve postoperatif nörolojik ve solunum sisteminin değerlendirilmesinin yapılıp bulguların kaydedilmesi, anestezi ve cerrahinin MS üzerindeki etkilerinin değerlendirilmesi açısından önemlidir. Hastaların kullandığı farmakolojik tedavi incelenmeli ve gerekli ise replasman tedavileri yapılmalıdır. Hastaların vücut ısıları sürekli monitorize edilmeli ve hipertermiden kaçınılmalıdır. Bu derlemede, multipl sklerozun etiyolojisi, ilerlemesi ve anestezik açıdan dikkat edilmesi gereken konular anlatılmaya çalışılmıştır.
