The relationship of demographic and etiological characteristics with mortality in acutely poisoned patients admitted to ıntensive care unit

Abstract

Aim: To evaluate the relationship of demographic and etiological charecteristics with mortality in acutely poisoned patients admitted to the intensive care unit (ICU).Material and Methods: Medical records of poisoned patients admitted to the ICU between January 2002 and July 2005 were reviewed retrospectively. Demographic data [age, sex, Glascow coma scale (GCS) on admission, APACHE II score], etiological data, and clinical outcome [length of stay (LOS) in the ICU, duration of mechanical ventilation, and the ICU mortality] were analyzed.Results: They were 121 poisoning patients (66 females and 55 males), and mean age was 38,15±14,34 years. GCS and APACHE II were 8,47±4,10 and 15,80±8,74 respectively. Toxic substances involved were, in descending order, organophosphates (45,5 %), tricyclic antidepressant (% 14,8), multiple drugs (% 14,8), methanol (% 9), different types of drugs (% 10,7), food (% 3,3), and carbon monoxide (% 1,6). The overall mortality rate was 15,7 %, and toxic agents caused the highest fatality rates were methanol and organophosphate (54,5 % and % 21,7, respectively). Young female were more poisoned with tricylic antidepressant and multiple drugs. Age, APACHE II, LOS in the ICU, and duration of mechanical ventilation were significantly higher in patients poisoned by methanol, while GCS was lower than other groups. Also, in nonsurvived patients age, APACHE II, and duration of mechanical ventilation were higher than survived patients, while GCS was lower than survived group.Conclusion: Age, GCS, APACHE II score, and the type of toxic substances may be a determinant for mortality in poisoned patients. While most of the poisonings were due to organophosphates, tricyclic antidepressant and multiple drugs were the most commonly ingested agents by young female.

Amaç: Yoğun bakım ünitesi (YBÜ)’ne kabul edilen akut zehirlenme olgularında bireysel ve etiyolojik özelliklerin mortalite ile ilişkisini değerlendirmek.Gereç ve Yöntem: Ocak 2002-Temmuz 2005 tarihleri arasında yoğun bakım ünitesine kabul edilen zehirlenme olgularının tıbbi kayıtları retrospektif olarak gözden geçirildi. Bireysel veriler [yaş, cins, başvurudaki Glaskow koma skalası (GKS) skoru, APACHE II skoru], etiyolojik veriler ve klinik sonuç [YBÜ’nde kalış süresi (YBKS), mekanik ventilasyon süresi ve YBÜ mortalitesi] incelendi. Bulgular: Yüz yirmi bir zehirlenme hastası (66 kadın, 55 erkek) vardı ve bu hastaların ortalama yaşları 38,15 ±14,34 idi. GKS ve APACHE II sırası ile 8,47±4,10 ve 15,80±8,74 olarak bulundu. Zehirlenmeye yol açan ajanlar, azalan sıra ile bildirildiklerinde, organofosfatlar (% 45,5), trisiklik antidepressanlar (% 14,8), birden fazla ilaç (% 14,8), metanol (% 9), farklı tipte ilaç (% 10,7), gıda (% 3,3) ve karbon monoksitten (% 1,6) oluşmaktaydı. Genel mortalite oranı 15,7% idi ve en yüksek fataliteye yol açan toksik ajanın da metanol ve ardından organofosfatlar (sırası ile % 54,5 ve % 21,7) olduğu saptandı. Trisiklik antidepressanlar ve birden fazla ilaçlarla zehirlenmeler daha çok genç kadınlarda görüldü. Metanol zehirlenmesi olanlarda, APACHE II, YBÜ’de kalış süresi diğer gruplardan önemli derecede daha yüksek ve GKS ise daha düşük bulundu. Ayrıca, yaşamayan hastalarda yaş, APACHE II ve mekanik ventilasyon süresinin yaşayanlardan daha yüksek, GCS’nin ise daha düşük olduğu saptandı.Sonuç: Yaş, GKS, APACHE II skoru ve toksik maddenin tipi, zehirlenen hastalarda mortalite belirleyicisi olabilir. Zehirlenmelerin çoğu organofosfatlarla gelişmiş olmakla beraber, trisiklik antidepressanlar ve birden fazla ilaç içimine genç kadınlarda daha sık rastlandı.
