Abstract
Awareness with recall after surgery under general anesthesia is an infrequent experience, but it may be associated with significant adverse psychological sequelae. The feelings of these patients are dominated by auditory perception, anxiety, panic, helplessness, sensations of being paralysed, and inability to tell the anesthesiologist about the perceived pain. Fortunately, the incidence of awareness with recall of severe pain is low. Most cases of awareness are inconsequential, but some patients may experience prolonged and unwanted outcomes, including nightmares, anxiety and daytime flashbacks. In a small proportion of these patients post-traumatic stress disorder associated with anxiety, irritability and fear from death might develop. Nowadays as many as one or two cases in every 1000 patients who receive general anesthesia for non-obstetric and non-cardiac surgery experience this outcome. Certain procedures are associated with increased risk, including cesarean section, cardiac surgery, and trauma surgery. While lower doses of general anesthetics can be predisposing factors, the incidence of awareness may be higher in patients requiring smaller doses of general anesthetics, and patients anesthetized with opioids, benzodiazepines, or nitrous oxide, given alone or in combination. In contrast, some researchers also suggested that addition of volatile anesthetics, as well as potent intravenous anesthetics such as, thiopental, etomidate and propofol in appropriate concentrations to these combinations might reduce the risk of intraoperative awareness. In this paper, risk factors, methods aiming at detection of risk factors for the emergence of intraoperative awareness, approaches for the prevention or reduction of the frequent occurrence of awareness, and the management of cases who manifested intraoperative awareness were summarized.

Genel anestezi altında uyanıklık, sık görülmeyen ancak, ciddi psikolojik sonuçları olabilen bir deneyimdir. Anestezi sırasında uyanıklık veya anımsama deneyimi yaşayan hastalar bu deneyimi sık olarak işitsel algılama, anksiyete, panik, yardım alamama, paralizi hissi ve anesteziste ağrı duyduğunu söyleyememe şeklinde tanımlamaktadır. Ağrı hissi ise oldukça enderdir. İntraoperatif uyanıklık yaşayan olguların bir kısmında daha sonra geceleri uyku bozukluğu, rüya ve kâbuslar, gündüzleri ise geçmişe dönük anımsamalar ve anksiyete yakınmaları olmaktadır. Çok küçük bir grupta ise anksiyete, irritabilite ve ölüm kaygısının eşlik ettiği posttravmatik stres sendromu gelişmektedir. Günümüzde genel anestezi altında uyanıklık sıklığının non-obstetrik ve non-kardiyak cerrahide % 0,1–0,2 olduğu kabul edilmektedir. Sezaryen, kardiyak cerrahi ve majör travma cerrahisi bu riski artırabilmektedir. Genel anesteziklerin düşük dozlarda kullanılması predispozan bir faktör olabileceği gibi; opiyoidler, benzodiyazepinler ve azot protoksitin tek başına ya da birlikte kullanıldığı olgularda bu oran daha yüksek olabilmektedir. Bu kombinasyonlara volatil anestezik ya da yeterli dozda tiyopental, etomidat ve propofol gibi potent intravenöz anesteziklerin eklenmesi ile intraoperatif uyanıklık riskinin azaltılmasının olası olabileceğini ileri süren çalışmacılar da bulunmaktadır. Bu makalede; erişkin olgularda intraoperatif uyanıklık gelişimi için risk faktörleri, intraoperatif uyanıklık gelişimini saptamaya yönelik yöntemler, uyanıklığı önleyecek ya da sıklığını azaltacak yaklaşımlar ile intraoperatif uyanıklık gelişen olguların yönetimi özetlenecektir.
