The effect of systemic ondansetron on subarachnoid block

Abstract
Aim: Ondansetron is a selective 5-hydroxytriptamin antagonist. It was shown that ondansetron has local anesthetic actions but it also antagonizes the sensorial block level of subarachnoidal lidocaine anesthesia Our aim was to evaluate whether intravenous ondansetron had an effect on the characteristics of subarachnoid block induced with bupivacaine+fentanyl and on the time to the first analgesic requirement during postoperative period. Material and Method: Forty-two ASA I-III patients scheduled for inguinal herniorraphy were included in the study. Fifteen minutes before subarachnoidal injection Group I (n=21) received ondansetron (8 mg iv) and Group II (n=21) an equal volume of 0.9 % normal saline solution. Subarachnoidal block was achieved with combined spinal-epidural anesthesia technique, and 5 mg 0.5 % bupivacaine+25 µg fentanyl+1.5 mL distilled water were injected intrathecally. Haemodynamic, sensorial and motor block parameters and side effects were assessed in all groups.Results: Demographic data, haemodynamic parameters, side effects, time to reach T6 dermatome and two segment regression time of the cases were similar. S2 regression and total anesthesia time were shorter in the ondansetron group but motor block parameters were similar between the groups. Conclusion: Although we concluded that intravenous ondansetron antagonizes sensorial block of spinal anesthesia induced with bupivacaine and fentanyl, further studies are needed to evaluate the mechanism of its effect on nociception and its interaction with other local anesthetics.
Özet
Amaç: Ondansetron selektif bir 5-hidroksitriptamin antagonistidir. Ondansetronun lokal anestezik aktiviteye sahip olduğu, ancak lidokain ile gerçekleştirilen subaraknoid anestezide sensoryal blok düzeyini antagonize ettiği gösterilmiştir. Çalışmanın amacı, intravenöz ondansetronun, bupivakain+fentanil kullanılarak oluşturulan subaraknoid blok karakteristikleri ve postoperatif dönemde ilk analjezik gereksinim zamanı üzerine etkisi olup olmadığını araştırmaktır.Gereç ve Yöntem: Çalışmamıza, inguinal herniografi operasyonu yapılacak ASA I-III grubundan 42 olgunun alınması planlandı. Grup I’de (n=21) subaraknoid enjeksiyondan 15 dk. önce 8 mg iv ondansetron, Grup II’de (n=21) eşit volümde % 0,9 serum fizyolojik iv verildi. Subaraknoid blok kombine spinal-epidural anestezi ile uygulandı. İntratekal 5 mg % 0,5 bupivakain+25 µg fentanil+1,5 mL distile su enjekte edildi. Tüm olgulara ait; hemodinamik parametreler, başlangıç zamanı, maksimum düzey, gerileme zamanı ve izlenen yan etkiler kaydedildi.Bulgular: Olguların bireysel verileri, hemodinamik parametreler, yan etkiler, T6 dermatomuna ulaşma zamanı ve iki segment regresyon zamanı benzer bulundu. S2 regresyon ve total anestezi zamanı ondansetron grubunda daha kısa saptanmasına karşın, motor blok parametreleri her iki grupta benzer bulundu.Sonuç: Çalışmamızda; subaraknoid enjeksiyondan önce iv olarak kullanılan ondansetron, bupivakain ve fentanil ile oluşturulan sensoryal bloğu antagonize etmesine karşın, ondansetronun nosisepsiyon üzerine etki mekanizmasını ve diğer lokal anesteziklerle ilişkisini belirlemek için ileri çalışmalara gereksinim olduğu kanısına varıldı.
