Incidences of venous air embolism in the sitting position: An analysis of 185 cases

Abstract

Aim: There are many advantages of sitting position in terms of surgical access into posterior fossa and also for cervical spine surgery. The main concern is the risk of venous air embolism (VAE). Our aim was to evaluate the incidence of VAE in patientsMaterial and Methods: All of the patients who underwent surgery in the sitting position performed by the Dept. of Neurosurgery, between 2004-2008 were enrolled in our prospective study. For the monitorization of all patients, central venous catheter was inserted into the right atrium by an electrocardiography guided technique. Monitoring for VAE were done by precordial Doppler (PD) and capnographic examinations. Diagnosis of VAE was suspected; when precordial Doppler tone was changed, and at least one of the following criteria was fullfilled: sudden decrease in end-tidal carbon dioxide pressure (≥ 3 mmHg) and/ or sudden decrease (≥ 20 %). in mean arterial pressure. Results: Seventy-eight men and 107 women (mean age 44.56±16.3 years) were enrolled in the study. VAE was detected in 51 of 185 operations and occured more than once during the same procedure in 13 patients. There was a total amount of 75 separate episodes of VAE. In 48 (94.1 %) patients air was aspirated through central venous catheter. The incidence of associated hypotension was 25.5 % (n=13). Sixteen (31,3 %) patients’ position was changed from sitting to prone position during the surgery. All VAE episodes responded promptly to treatment and there was no peroperative morbidity or mortality attributed to VAE. Conclusion: In the sitting position VAE can occur in every step of the surgery, most frequently during craniotomy and tumor resection procedures. Especially during these periods patients must be monitored for VAE. VAEs can be quickly detected with preemptive placement of a precordial Doppler probe. Early detection of VAE can expedite therapeutic maneuvers, such as elimination of the source of VAE.

Amaç: Oturur pozisyonun, posteriyor fossa ve servikal vertebra cerrahisinde cerrahi girişimi kolaylaştırdığı için birçok avantajı vardır. Temel kaygı venöz hava embolisi (VHE) riskidir. Çalışmada; hastanemizde, oturur pozisyonda posteriyor fossa ve servikal vertebra cerrahisi yapılan hastalarda VHE insidansının belirlenmesi amaçlanmıştır. Gereç ve Yöntem: Prospektif düzenlenen çalışmamıza, 2004-2008 tarihleri arasında, beyin cerrahisi kliniği tarafından oturur pozisyonda operasyona alınan hastaların tamamı alındı. Tüm hastalarda elektrokardiyografi ve kılavuz yöntemi ile sağ atriyuma yerleştirilen santral venöz kateteri ile izlem gerçekleştirildi. VHE izlemi için prekordiyal Doppler ve kapnografi monitorizasyonu yapıldı. VHE tanısı; prekordiyal Doppler cihazında ses değişikliği saptanması, end-tidal karbondioksitte 3 mmHg’dan fazla azalma olması ya da ortalama kan basıncında % 20’den fazla ani azalma ölçütlerinden en az birinin bulunması ile konuldu. Bulgular: Ortalama yaş 44,56±16,3 yıl olan 78 erkek, 107 kadın toplam 185 olgu çalışmaya alındı. VHE 185 operasyonun 51’inde saptandı ve 13 olguda aynı operasyon sırasında birden fazla oluştu. Toplam 75 ayrı VHE atağı görüldü. Olguların 48 (% 94,1)’inde santral kateterden hava aspire edildi. On üç olguda hipotansiyon görüldü (% 25,5). VHE tedavisi amacıyla 16 (% 33,1) olgunun pozisyonu supinden pron pozisyonuna, cerrahi girişim sırasında değiştirildi. Tüm VHE atakları tedaviye yanıt verdi, VHE ile ilişkili peroperatif morbidite veya mortalite gözlenmedi.Sonuç: Oturur pozisyonda VHE, cerrahinin her döneminde görülebildiği gibi en sık kraniyotomi ve tümör rezeksiyonu evresinde görülmüştür. Özellikle bu dönemler VHE embolisi açısından sıkı izlenmelidir. VHE, önceden prekordiyal doppler probunun yerleştirilmesi ile hızlı saptanabilir. VHE’nin erken saptanması, embolinin eliminasyonu gibi tedavi edici girişimleri hızlandırabilir.
