Anesthetic management of simultaneous carotid endarterectomy, coronary revascularization, aortic-mitral valve replacement and atrial septal defect repair surgery

Abstract
Nowadays the incidence of simultaneous interventions in open heart surgery increases. In this report, we are presenting anesthetic management of an elderly patient who underwent simultaneous carotid endarterectomy, multivessel complete coronary revascularization, aortic-mitral valve replacement and atrial septal defect repair. A 75-year-old female patient was referred to cardiovascular surgery clinic for surgical treatment of severe carotid stenosis, aortic and mitral valve disease with multivessel coronary artery involvement. Primarily, left carotid endarterectomy was performed with continuous electroencephalographic (EEG) and cerebral oximetry (NIRS) monitoring. After double clamping, carotid plaque was removed and then the artery was closed primarily. Clamping lasted for 4 minutes and a shunt was not inserted. Meanwhile EEG and NIRS recordings remained unchanged. Priory 4-vessel coronary grafting and then aortic and mitral valve replacements were performed. After left atriotomy, a secundum atrial septal defect was detected which was closed primarily. Effective cardiac and cerebral protection was achieved during the operation. Per and postoperatively, the patient did not have any complication. This case showed that combined cardiac operations could be performed safely with carefully and satisfactorily monitored anesthetic approach in high-risk patients.

Açık kalp cerrahisinde kombine operasyonlar günümüzde sıklıkla uygulanmaktadır. Bu yazıda tek seansta sol karotis endarterektomi, 4 damar koroner baypas, aort - mitral kapak replasmanı ve atriyal septal defekt onarımı yapılan ileri yaştaki bir hastanın anestezi yönetimi sunulmaktadır. Yetmiş beş yaşında kadın hastaya sol karotis endarterektomi, koroner revaskülarizasyon, aort ve mitral kapak replasmanı planlandı. Önce sol karotis endarterektomi yapıldı. Sürekli elektroensefalografi (EEG) ve serebral oksimetri (NIRS) izlemi altında karotise klemp kondu ve ülsere plak temizlenip damar primer kapatıldı. Karotis klempi 4 dk. sürdü. Şant kullanılmadı. Bu sırada, EEG ve NIRS değerlerinde değişiklik olmadı. Hastaya önce 4’lü koroner greftleme ardından çift kapak replasmanı yapıldı. Bu aşamada kalp boşlukları açıldığında sekundum tip atriyal septal defekt olduğu görüldü ve primer onarıldı. Operasyon süresince etkin bir kalp ve beyin korunması uygulandı. Operasyon sırasında ve sonrasında herhangi bir komplikasyon olmadı. Bu olgu, dikkatli ve iyi monitörize edilerek uygulanan bir anestezi yönetimiyle, eşzamanlı kardiyak operasyonların riskli hastalarda güvenle yapılabileceğini düşündürmektedir.
