The effects of peroperative dexmedetomidine infusion on thiopental need, haemodynamics, first analgesic requirement and recovery responses

Abstract
Aim: In this placebo-controlled trial we aimed to compare the efficacy of dexmedetomidine loading dose followed by infusion immediately after anaesthesia induction on thiopental consumption at induction, intraoperative hemodynamics, first postoperative analgesic requirement and recovery. Material and Method: Thirty patients in this prospective, randomized, double blind, placebo- controlled trial were allocated into two groups. Group I received dexmedetomidine 1 μg kg-1 iv over 10 minutes before induction, followed by an infusion of 0.4 μg kg-1 h-1. Group II was given the same dose of isotonic NaCl 0.9 %. Thiopental amount required to decrease BIS to 40 at induction, mean arterial blood pressure (MAP), heart rate (HR) at 5, 10. min before induction; after induction, before and after intubation; intraoperative 5., 10., 20., 30., 40. min; before and after extubation and at 15 min intervals for the first postoperative hour, at every hour for the rest four hours were recorded while comparing early and late Aldrete recovery scores, postoperative first analgesic requirement and side effects. Results: Thiopental dose at induction was lower in Group I. MAP was higher in Group I at 5. min of preoperative infusion, but lower at 20., 30., 40 min intraoperatively, after extubation, 30., 180., 240. min postoperatively. Amaç: Bu çalışmanın amacı, yükleme dozu ve anestezi indüksiyonu sonrası başlayan deksmedetomidin infüzyonunun; indüksiyonda tiyopental gereksinimi, intraoperatif hemodinami, postoperatif ilk analjezik gereksinimi ve derlenme üzerine olan etkilerini plasebo ile karşılaştırmaktır. Gereç ve Yöntem: Prospektif, randomize, çift kör, plasebo kontrollü bu çalışmaya katılan 30 hasta 2 gruba ayrıldı. Birinci gruptaki hastalara (Grup I) anestezi indüksiyonu öncesinde 10 dk.’lık 1 μg kg-1 iv deksmedetomidin yükleme dozu verildikten sonra anestezinin sürdürülmesinde 0,4 μg kg-1 sa-1 infüzyonu uygulanırken, ikinci gruptaki hastalara (Grup II) aynı süre ve dozlarda % 0,9 izotonik NaCl uygulandı.
