The effcacy of needle-free injection system (INJEX®) in relieving the pain of spinal needle insertion

Abstract

Aim: INJEX® is a needle-free drug delivery system mainly developed for vaccination and insulin applications. It hasbeen successfully used in various anesthesia procedures. This study was designed to compare the effects of localanesthesia application with INJEX®, a needle free injection system or with dental needle on relieving pain associatedwith spinal needle insertion and relevant patient satisfaction. Material and Methods: Sixty patients scheduled for surgical operations under spinal anesthesia were randomlyallocated to two groups. Prior to spinal anesthesia, spinal needle insertion area was infltrated with 1.5 mL of 2 %lidocaine using 27 gauge dental needle in Group 1 and 0.3 mL, 2 % lidocaine by needle-free injection system (INJEX®model 210) in Group 2. Pain intensities during local anesthetic application and spinal needle insertion were assessedimmediately after each procedure by visual analogue scale (VAS). Patient satisfaction in each group was also comparedon a 4-point scale.Results: Pain scores measured during local anesthesia administration were signifcantly lower in the INJEX® groupthan in the dental needle injection group (p=0.001). No signifcant difference was found between two groups as forpain scores during spinal needle insertion (p=0.382). Patient satisfaction was found to be higher with INJEX® with alimited signifcance (p=0,049). Conclusion: INJEX® is an effective alternative to dental needle for lidocaine infltration during the administration ofspinal anesthesia with less pain and higher patient satisfaction.

Özet

Amaç: INJEX®, başlangıçta aşı ve insülin uygulamalarında kullanılmak amacıyla geliştirilmiş iğnesiz enjeksiyon sistemidir. Günümüzde farklı anestezi uygulamalarında da başarılı olarak kullanılmaktadır. Bu çalışmada, spinal anesteziöncesi iğnesiz enjeksiyon sistemi (INJEX®) ve iğne ile lokal anestezi uygulamasının, spinal iğne giriş ağrısı ve hastamemnuniyeti üzerine etkisinin karşılaştırılması amaçlandı. Gereç ve Yöntem: Spinal anestezi uygulanarak cerrahi girişim planlanan 60 hasta randomize olarak 2 gruba ayrıldı.Spinal anestezi işlemi uygulamasından hemen önce spinal iğnenin uygulanacağı bölgeye Grup 1 hastalara 27 Gaugedental iğne ile 1,5 mL % 2 lidokain, Grup 2 hastalara iğnesiz enjeksiyon sistemi (INJEX® model 210) ile 0,3 mL % 2lidokain kullanılarak infltrasyon anestezisi uygulandı. Lokal anestezik uygulaması ve spinal anestezi sırasındaki ağrıskorları VAS ile hasta memnuniyeti ise 4 nokta skalası ile karşılaştırıldı.Bulgular: Lokal anestezi uygulaması sırasında ağrı skorlarının değerlendirilmesinde, INJEX® kullanılan hastalardagörsel ağrı skalası (VAS) değerleri iğne ile uygulananlara göre düşük bulundu (p=0,001). Spinal iğne giriş ağrısında ikigrup arasında anlamlı bir farklılık saptanmadı (p=0,382). İstatistiksel olarak sınırda anlamlı olmakla beraber, INJEX® ilehasta memnuniyetinin daha fazla olduğu saptandı (p=0,049). Sonuç: Spinal anestezi öncesi INJEX® ile lokal anestezi uygulaması, dental iğne ile lidokain uygulamalarına göre dahaaz ağrılı işlem olup, hasta memnuniyetinin iyi olduğu alternatif bir yöntem olabileceği değerlendirildi.
