Comparison of the effects of lidocaine, ketamine and ephedrine in the prevention of injection site pain of rocuroniu

Abstract

Aim: Rocuronium can cause pain on injection in large population of patients and various methods have been studiedto reduce this pain. In this study, the effectiveness of lidocaine, ketamine and ephedrine in the prevention of rocuronium injection site pain were compared.Materials and Methods: This prospective, randomized study was carried-out on 120 patients after obtaining theapproval of the Ethics Committee. Patients were randomized into four groups and lidocaine 1 mg kg-1(Group L, n=30),ketamine 0.2 mg kg-1(Group K, n=30), ephedrine 70 µg kg-1(Group E, n=30), normal saline 5 mL (Group S, n=30)were given intravenously prior to rocuronium administration. Rocuronium pain was assessed using verbal pain scoreand withdrawal response score. Results: Verbal pain scores of 0 (no pain) in groups L, K, E and S were estimated in 80 %, 50 %, 20 %, 20 % of thepatients, respectively. Withdrawal response scores of 0 (none) and 1 (withdrawal response only in wrist) in groups L,K, E and S were noted in 93.3 %, 80 %, 73.3 %, and 46.7 of the cases, respectively. Conclusion: A signifcant reduction was found in incidence of pain and withdrawal response in both lidocaine andketamine groups compared with ephedrine and saline groups. Despite ephedrine was found ineffective in reduringverbal pain scores, it is more effective than saline in reduring withdrawal score.

Amaç: Rokuronyum enjeksiyonu, birçok hastada ağrıya yol açabilir ve önlenmesi için çeşitli yöntemler denenmiştir.Bu çalışmada, rokuronyumun enjeksiyon ağrısının önlenmesinde lidokain, ketamin ve efedrinin etkinliği karşılaştırılmıştır. Gereç ve Yöntem: Prospektif, randomize olan bu çalışma, etik kurul onayı alındıktan sonra, 120 hasta üzerinde yapıldı. Rokuronyum verilmeden önce, lidokain 1 mg kg-1(Grup L, n=30), ketamin 0.2 mg kg-1(Grup K, n=30), efedrin70 µg kg-1(Grup E, n=30), serum fzyolojik 5 mL (Grup S, n=30), dört gruba ayrılan hastalara uygulandı. Rokuronyumağrısı, sözel ağrı skoru ve çekme yanıtı skoru ile değerlendirildi.Bulgular: Sözel ağrı skoru 0 (yok) olan hastalar Grup L, K, E ve S’de sırasıyla % 80, % 50, % 20, % 20 oldu. Geriçekme skoru 0 (yok) ve 1 (sadece el bileğinde çekme yanıtı) olan hastalar Grup L, K, E ve S’de sırasıyla % 93.3, % 80,% 73.3, % 46.7 olarak gözlemlendi. Sonuç: Lidokain ve ketamin gruplarında, sözel ağrı ve geri çekme skorlarında efedrin ve kontrol gruplarına görebelirgin bir azalma saptandı. Efedrin sözel ağrı skorlarının azaltılmasında etkisiz bulunmuş ise de geri çekme skorlarınınazaltılmasında serum fzyolojikten daha etkindi.
