Intracranial arterio-venous malformation surgery lasting 34 hours

Abstract

A 30 year- old man with convulsions was diagnosed as arterio-venous malformation (AVM) on magnetic resonanceimaging (MRI). Surgical extirpation of AVM was performed under general anesthesia. The duration of the operationwas 34 hours. He was intubated when admitted to the intensive care unit following surgery and he was extubated 1hour after his admission into ICU. When he was transferred to the ward on 3rd postoperative day his Glasgow ComaScore was 15 and his muscle power was full. In this case report we aimed to discuss surgical and anesthetic complications in long- lasting operations and our management principles.

Özet

Otuz yaşında erkek hasta konvülsiyon yakınmasıyla nöroşirurji servisine başvurmuş, yapılan manyetik rezonans görüntüleme (MRG) ve anjiyograf ile arteriyovenöz malformasyon (AVM) görülmüştür. Hasta AVM eksizyonu amacıylagenel anestezi uygulanarak ameliyata alınmış, ameliyatı 34 saat sürmüştür. Ameliyat bitiminde yoğun bakım ünitesinealınmış 1 saat sonra ekstübe edilip, postoperatif 3. günde bilinç açık koopere, Glasgow Koma Skoru 15 ve ekstremitelerinde de kas gücü tam olarak servise gönderilmiştir. Bu olgu sunumunda, uzun süreli ameliyatlarda; cerrahi veanesteziye bağlı gelişebilecek komplikasyonlar ile uyguladığımız tedavileri değerlendirmeyi amaçladık.
