Abstract

Objective: Our aim was to compare the effects of subcutaneous 2 % lidocaine infltration and topically appliedlidocaine-prilocaine cream (EMLA®) on perception of pain, complications, patient and physician satisfaction in patientsscheduled for cardiovascular surgery who were to receive invasive arterial monitoring.Materials and Methods: The study included 128 ASA II-III patients aged 40-80 years who were randomized to receive either 0.5 mL 2 % lidocaine infltration 5 minutes before the radial artery cannulation (Group L) or 1 mL of EMLA®cream 1 hour before the attempt (Group E). Before, during and after the cannulation, noninvasive systolic, diastolic andmean arterial pressures (SAP, DAP, MAP), peripheral oxygen saturation (SpO2 ), respiration rates (RR), and heart rates(HR) were recorded. Visual Analogue Scale (VAS) scores were noted during the frst (VAS-1) and ffth minute (VAS-2)after the completion of cannulation. Thereafter, number of attempts, complications, patient and physician satisfactionwere evaluated. Results: No signifcant difference was found between the two groups in terms of individual data; HR, SAP, DAP,MAP, SpO2and RR values, as well as the number of attempts. In both groups, a decrease was established in VAS-2scores as compared to VAS-1 (p

Özet

Amaç: Kardiyovasküler cerrahi planlanan ve invaziv arter monitörizasyonu yapılacak hastalarda subkutan % 2 lidokain infltrasyonu ile topikal uygulanan lidokain-prilokain kremin (EMLA®); ağrı, komplikasyonlar, hasta ve hekimmemnuniyeti üzerindeki etkinliklerinin karşılaştırılması amaçlandı.Gereç ve Yöntem: Çalışmaya radiyal arter kanülasyonu işleminden 5 dk. önce 0,5 mL % 2 lidokain infltrasyonu (GrupL) veya girişimden 1 saat önce 1 mL EMLA® krem (Grup E) uygulanmak üzere rasgele seçilen 40-80 yaş arası 128 ASAII-III grubu hasta alındı. Kanülasyon öncesinde, sırasında ve sonrasında noninvaziv sistolik, diyastolik ve ortalama arterbasıncı (SAB, DAB, OAB), periferik oksijen saturasyonu (SpO2 ), solunum sayısı (SS) ve kalp atım hızı (KAH) kaydedildi.Vizüel Analog Skala (VAS) skorları kanülasyon tamamlandıktan sonra 1. (VAS-1) ve 5. (VAS-2) dk.’larda not edildi. Dahasonra, girişim sayısı, komplikasyonlar, hasta ve hekim memnuniyeti değerlendirildi.Bulgular: Bireysel verilerde, KAH, SAB, DAB, OAB, SpO2ve SS değerlerinde ve girişim sayısında iki grup arasındaanlamlı bir fark bulunmadı. Her iki grupta VAS-2 skorlarında VAS-1’e göre azalma saptandı (p
