Fiberoptic bronchoscope versus lightwand guided techniques for percutaneous dilational tracheostomy

Abstract

Aim: In recent years, percutaneous tracheostomy (PT) has become a routine practice for ICU patients requiringprolonged mechanical ventilation. Complications of PT include bleeding, loss of airway control, inadvertent injury tosurrounding structures and equipment damage, and all of them can be attributed to poor visualization and inaccurateorientation. In this study we aimed to compare the effectiveness of lightwand or fberoptic bronchoscope (FB)-guidedtechniques in percutaneous dilational tracheostomy (PDT). Material and Methods: In a randomised design, between June 2007 and February 2009, 30 consecutive PDT procedures were performed with FB-guided technique in group 1 and with lightwand-guided technique in group 2 onadult ICU patients. The Griggs technique and Portex set were used during procedures. All procedures were performedby the same anesthesiologist experienced with the PDT technique. Demographic data, APACHE II scores, duration ofmechanical ventilation, and procedure, arterial blood gas analysis, bleeding, and complication rates were recorded. Results: We haven’t observed any mortality due to PDT in our cases and there were no differences in complicationrates and other data except duration of PDT. Bleeding was observed at a rate of 10 % only in FB-guided tracheostomygroup. Conclusion: The lightwand- guided PDT is a simple, effective and a safe procedure. This technique can avoid therisk of puncturing the endotracheal tube and/or cuff, allowing adequate ventilation and oxygenation during PDT.Furthermore, this technique is inexpensive and minimizes the risk of damaging equipment like FB-guided technique.However, larger patient series is necessary to decide exactly which technique will be safer.

Amaç: Son yıllarda, perkütan trakeostomi (PT) uzun süre mekanik ventilasyon gereken YB hastaları için rutin bir uygulama olmuştur. PT’nin komplikasyonları kanama, hava yolu kontrolünün kaybı, çevre yapılarının istenmeyen hasarı,malzeme hasarı, zayıf görüş ve yetersiz oryantasyonu içerir. Bu çalışmada, perkütanöz dilatasyonal trakeostomi (PDT)yönteminde ışıklı stile veya fberoptik bronkoskopi (FB) kullanımının hemodimamik değişiklikler ve komplikasyonlarüzerine etkilerini karşılaştırmayı amaçladık.Gereç ve Yöntem: PDT işlemi Haziran 2007 ve Şubat 2009 döneminde yoğun bakım ünitesinde yatmakta olan 30erişkin hastada rasgele seçimle gerçekleştirildi. PDT yöntemi Grup 1’de FB eşliğinde, Grup 2’de ise ışıklı stile eşliğindeuygulandı. İşlem süresince, Griggs yöntemi ve Portex set kullanıldı. Tüm işlemler PDT yöntemi konusunda deneyimliaynı anestezist tarafından oluşturuldu. Bireysel veriler, APACHE II skorları, mekanik ventilasyon süresi, işlem süresi,arteriyel kan gaz analizleri, kanama ve komplikasyon oranları kaydedildi. Bulgular: Olgularımızda PDT nedeniyle herhangi bir mortalite gözlemedik ve PDT süreleri dışında, komplikasyonoranları ve diğer verilerde farklılık yoktu. Kanama, FB eşliğinde trakeostomi grubunda % 10 ve ışıklı stile eşliğindetrakeostomi grubunda % 0 gözlendi.Sonuç: Işıklı stile eşliğinde PDT basit, efektif ve güvenli yöntemdir. Bu yöntem, entübasyon tüpünün ve/veya kafınındelinmesi riskini önleyebilir, böylece PDT süresince yeterli ventilasyon ve oksijenasyona izin verir. Bu yöntem ucuzdurve FB gibi cihaz hasarlanmalarının riskini azaltır. Ancak, yöntemin güvenli olacağına tam olarak karar vermek için dahageniş hasta serileri gereklidir.
