Tracheal tube damage due to harmonic scalpel use during tonsillectomy: Case report

Abstract

Harmonic scalpel is used frequently in recent years because it reduces intraoperative tonsillectomy bleeding, postoperative pain and shortens the operation time. Harmonic scalpel is a surgical tool which is activated ultrasonically andused for cutting and coagulation at low temperatures (50-100°C). While the device is active, it is necessary to avoidcontact with any metal, plastic or explosive/fammable materials because the long-term use would increase the risk ofthermal damage In this paper we reported tracheal tube damage and airway management diffculties due to the useof harmonic scalpel.

Özet

Tonsillektomilerde kanama, postoperatif ağrı ve ameliyat süresini kısalttığı için son yıllarda sıklıkla kullanılan harmonic scalpel, ultrasonik olarak aktive edilen düşük sıcaklıkta (50-100°C) kesme ve koagülasyon yapan cerrahi bir alettir.Cihaz aktif iken, herhangi bir metal veya plastik materyal ile temas ettirilmemesi ve uzun süreli kullanımda ısı miktarınınyükseleceğinden ve bu nedenle patlayıcı/yanıcı materyallerle temasından kaçınılması gerekmektedir. Bu olgu sunumunda, harmonic scalpel kullanımına bağlı trakeal tüp hasarı ve oluşturduğu hava yolu sorunları anlatılmıştır.
