Effects of sedation provided with midazolam or propofol on carotid blood flow

Abstract

Aim: The aim of our study is to compare the effects of sedation provided with propofol and midazolam on carotid blood flow during cataract surgery. Material and Method: Forty patients undergoing cataract surgery with local anesthesia were divided randomly into two groups. The patients was given midazolam 0.03 mg kg-1 or 0.8 mg kg-1 propofol iv in 30 seconds. Carotid blood flow velocity of the patients was measured before the sedation and, when the values of BIS were 80. Sedation scores, BIS value of 80 for the last periods, hemodynamic values including SpO2, blood pressure, pulse, ECG findings and carotid blood flow velocities were recorded. Results: The groups were not significantly different terms of demographic data. In peak systolic flows velocity, when BIS value of 80 pre-quarter propofol sedation group, 20 %, decreased by 18 % in the midazolam group in the intra-group comparison (p=0.001). The time between until BIS values are 80 and sedation scores of elapsed time to be a 2 was shorter in the propofol group (respectively, p=0.016 and p=0.013). Takes to get the 2 sedation scores and BIS values of 80 for the time elapsed between the high correlations were observed (r=0.928, p=0.001). The sedation scores when the BIS value was 80 were similar between groups. In both groups, the sedation scores of 2 or 3 the number of patients was detected as 15 (75 %). Conclusion: As a result, sedation applied with propofol and midazolam reduces the cerebral blood flow but does not have any differences between the two drugs.

Amaç: Çalışmamızın amacı, katarakt operasyonu sırasında, propofol ve midazolam ile sağlanan sedasyonun karotis kan akımına etkilerinin karşılaştırılmasıdır. Gereç ve Yöntem: Lokal anestezi ile katarakt operasyonu geçirecek 40 hasta rasgele iki gruba ayrıldı. Hastalara 0,03 mg kg-1 midazolam ya da 0,8 mg kg-1 propofol 30 saniye içinde iv verildi. Sedasyon öncesinde ve BİS değerleri 80 olduğunda hastaların karotis kan akım hızları ölçüldü. Sedasyon skorları, BİS değerinin 80 olması için geçen süreler, SpO2, kan basıncı, nabız, EKG bulgularını içeren hemodinamik değerler ve karotis kan akım hızları kaydedildi. Bulgular: Gruplar arasında bireysel veriler açısından anlamlı bir fark yoktu. Grup içi karşılaştırmada pik sistolik akım hızında, BİS değeri 80 olduğunda sedasyon öncesi döneme göre propofol grubunda % 20, midazolam grubunda % 18 oranında azalma oldu (p=0.001). BİS değeri 80 olana kadar geçen süre ile sedasyon skorunun 2 olmasına kadar geçen süre propofol grubunda daha kısa idi (sırasıyla; p=0.016 ve p=0.013). Sedasyon skorunun 2 olması için geçen süre ile BİS değerinin 80 olması için geçen süre arasında yüksek korelasyon gözlendi (r=0.928, p=0.001). BİS değeri 80 olduğu andaki sedasyon skorları her iki grupta da benzerdi. Her iki grupta da sedasyon skorunun 2 ya da 3 olduğu hasta sayısı 15 (% 75) olarak saptandı. Sonuç: Sonuç olarak, katarakt operasyonu sırasında propofol ve midazolam ile uygulanan sedasyon beyin kan akımını azaltmaktadır, ancak her iki ilacın etkileri arasında fark bulunmadığı değerlendirilmiştir.
