Retrospective evaluation of double nerve stimulation technique for sciatic nerve block by popliteal approach

Abstract
Aim: Sciatic nerve block with double nerve stimulation is a technique that provides both rapid motor block and ahigher rate of success. In this study, we retrospectively evaluated the patient satisfaction, and effciency, and complications of this method in order to demonstrate its success and reasons for its preference during routine applications. Material and Method: ASA-I/III 106 patients undergoing foot and ankle surgery under sciatic nerve blockade bydouble stimulation technique were evaluated retrospectively. Sciatic block was performed with a 100 mm Stimuplex® needle and a nerve stimulator in all patients. The stimulator was set to deliver pulses at 1 mA, 2 Hz frequency for0.1 ms. N.tibialis and N.peronealis were separately located at the point of motor response with a stimulating currentof 0.4mA and local anesthetic (LA) mixture was injected separately into each nerve (LA solution: equal volumes of 0.5% bupivacaine+ 2 % prilocaine, max. 30 mL). Demographic data, parameters of the blockade and related complications were recorded. Neurological complications during nerve blockade were evaluated 48 hours after the blockadeby phone interviews with the patients. Patient satisfaction and the anesthesiologist’s opinion about the block wereevaluated separately. Results: The block performance time (8±3 min); time to the onset of motor 14±6 min) and sensory blockade (16±5min), time for reversal of the motor (610±122 min), and sensory blockade (658±128 min) were recorded.. Ninety percent of the patients declared acceptance to have a similar operation with the same anesthetic method in the future.Conclusion: Sciatic nerve blockade by double stimulation technique via popliteal approach was found to be safeand useful as it provided a highly successful and effcient anesthesia, high patient satisfaction with negligible complications.

Amaç: Çift sinir uyarı yöntemi ile siyatik sinir blok uygulaması, hem blok başarısını artırmakta hem de hızlı blok oluşumu sağlamaktadır. Bu çalışmada, bu yöntemin rutin uygulamadaki başarısı, hasta memnuniyeti, etkinliği ve komplikasyonları geriye dönük olarak değerlendirilerek seçilebilirliğinin ortaya konması amaçlanmıştır. Gereç ve Yöntem: Çift sinir uyarı yöntemiyle oluşturulan siyatik sinir bloğu ile ayak ve ayak bileği cerrahisi geçirenASA I-III 106 hastanın kayıtları geriye dönük olarak incelendi. Tüm hastalara 100 mm Stimuplex® iğne ile sinir stimulatörü kullanılarak, stimulatör 1 mA akımda, 2 Hz frekans ve 0,1 ms hızda ayarlanarak tibiyal ve peroneal motor siniryanıtları alındığında sinir stimulatörünün uyarısı 0,4 mA akıma düşürülerek lokal anestezik verildiği saptandı (% 0,5 bupivakain 15 mL + % 2 prilokain 15 mL olarak maksimum 30 mL). Hastalara ait bireysel veriler, blokla ilgili parametrelerve komplikasyonlar kaydedildi. Blok sırasında olabilecek nörolojik komplikasyonların bloktan 48 saat sonra hastalarlatelefonla görüşülerek değerlendirildiği görüldü. Hastaların memnuniyeti ve bloğu uygulayan anestezistin blokla ilgiligörüşleri ayrı ayrı değerlendirildi.Bulgular: Blok uygulama zamanı 8±3 dk., motor blok oluşum zamanı 14±6 dk., duysal blok oluşum zamanı 16±5dk., motor blok çözülme zamanı 610±122 dk. ve duysal blok çözülme zamanı 658±128 dk. idi. Hastaların % 90’ı gelecekte aynı anesteziyle benzer ameliyat olmayı isteyebileceğini belirttikleri görüldü.Sonuç: Çift sinir uyarı yöntemiyle siyatik sinir bloğunun yüksek derecede başarılı anestezi ve hasta memnuniyetiniçok az komplikasyonla sağlaması açısından güvenli ve uygun olabileceği kanısına varıldı.
