An ischemic complication after radial artery canulation that results with an amputation

Abstract

The right radial artery of a 74-year old female patient, who was admitted to intensive care unit because of cardiac arrest, was cannulated after performing Allen’s test. In the second day following the cannulation, it was seen that, the hand was cyanotic and there was bulla formation, so the cannula was removed. Thrombolytic and anticoagulant treatment were started. The widespread cyanosis of her right hand was regressed, but necrosis was formed on the first and second fingers. As there was no response to the treatment, the first and second fingers were amputated three weeks later. The patient who had been monitored for the development of hypoxic ischemic encephalopathy (HIE) , died 62th days after her admission to intensive care unit because of cardiac causes.

Özet

Kardiyak arrest sonrası bilinci kapalı olarak mekanik ventilasyon desteği, yakın takip ve tedavi amacı ile yoğun bakım ünitesinde takip edilen 74 yaşındaki kadın hastaya Allen testi yapılarak sağ radiyal arterden kanül yerleştirildi. Arter kanülasyonunun 2. gününde 1. ve 2. parmakta daha belirgin olmak üzere sağ elin genelinde siyanoz ve büller oluşması üzerine kanül çıkartıldı. Trombolitik ve antiagregan tedavi başlandı. Sağ elin genelindeki siyanozun gerilemesine rağmen, ilk iki parmaktaki iskemik bulguların ilerlediği görüldü. Tedaviye yeterli yanıt alınamadı ve sağ el 1. ve 2. parmakta oluşan nekrozun demarkasyon hattının tamamlanması üzerine kanülasyondan 3 hafta sonra sağ el ilk iki parmak ampute edildi. Kardiyak arrest sonrası hipoksik iskemik ensefalopati gelişen hasta takibinin 62. gününde mevcut kardiyak sorunlar sonucu kaybedildi.
