Local anesthetic toxicity managed successfully with lipid Infusion (Case report)

Abstract
Local anesthetic (LA) toxicity is the most fatal complication of peripheral nerve block techniques. Inadvertent intravascular administration or higher dose LAs above safety margins are the main causes of toxicity. In recent years, some case reports have been published in the literature about successful treatment of LA toxicity unresponsive to standard resuscitation efforts with lipid infusions. We aimed to report about a patient who suffered from a LA toxicity after sciatic + femoral nerve block for the removal of an infected knee prosthesis who was successfully treated with lipid infusion before development of cardiac arrest.
Özet
Lokal Anestezik (LA) toksisitesi periferik sinir bloğu tekniklerinin mortalitesi en yüksek komplikasyonudur. LA’lerin yanlışlıkla intravasküler uygulanması ya da emniyet sınırının üzerindeki dozlarda kullanımı toksisitenin en sık nedenleridir. Son yıllarda lipid emülsiyonu infüzyonlarının standart resüsitasyon uygulamalarına yanıt vermeyen toksisite durumlarında başarıyla kullanıldığına dair olgu sunumları bildirilmektedir. Bu olgu sunumunda enfekte diz protezinin çıkarılması için siyatik + femoral sinir bloğu uygulanmasını takiben LA toksisitesi gelişen ve kardiyak arrest gelişmeden önce uygulanan lipit infüzyonu ile başarı ile tedavi edilen bir hastamızı sunmayı amaçladık.
