The effect of BIS monitorization on the consumption of sevoflurane in transsphenoidal hypophysectomy

Abstract

Aim: We investigated the impact of anesthetic depth by bispectral index (BIS) monitorization on anesthetic drug usage, eye opening and extubation time after anesthesia on patients who are undergoing transsphenoidal hypophysectomy under general anesthesia. Material and Method: After approval of Kocaeli University Ethics Committee 60 ASA II-III patients aged 18-60 years who were scheduled for transsphenoidal hypophysectomy were enrolled in this prospective randomized study. The patients were divided into 2 groups as Groups I, and II which included patients who were followed-up with or without BIS monitorization, respectively. After routine monitorization; Group I was monitorized by BIS before the induction of anasthesia. The same anesthesia technique was administered to all of the patients. In Group I, concentration of sevoflurane was regulated according to the BIS ratio (50-60). In Group II sevoflurane concentration was 2 %. Measurements: The parameters were recorded preoperatively, after induction, and at 5., 10., and 15. minutes post- intubation, and also every 15 minutes during the operation. The administration of the volatile agent was terminated, after completion of the surgical procedure. The period from the cessation of the anesthetic drugs up to the extubation was defined as the extubation time, and the time between verbal stimulus, and eye-opening was assessed as eye opening time. Besides total usage of sevoflorane and remifentanyl and value of the MAC were recorded at end of the operation. Results: Age, body weights, gender and total usage of the remifentanyl were similar in both groups. Amaç: Çalışmamızda, genel anestezi altındaki hastalarda anestezi derinliğinin, Bispektral İndeks (BİS) yoluyla takip edilmesinin kullanılan anestezik ilaç miktarlarına ve anestezi sonrası göz açma ve ekstübasyon sürelerine etkisini araştırdık. Gereç ve Yöntem: Bu prospektif randomize çalışmaya Kocaeli Üniversitesi Etik Kurul izni alındıktan sonra transsfenoidal hipofizektomi cerrahisi planlanmış, ASA II-III olan, 18–65 yaş arasındaki 60 hasta dahil edildi. Hastalar 2 gruba ayrıldı. Grup I: BİS monitörizasyonlu, Grup II: BİS monitörizasyonsuz. Rutin monitörizasyon sonrası Grup I’e indüksiyondan önce BİS monitörizasyonu uygulandı. Tüm hastalara aynı anestezi yöntemi uygulandı. Sevofluran konsantrasyonu Grup I’de BİS oranına (50–60) göre ayarlandı, Grup II’de % 2 konsantrasyonda tutuldu. Ölçümler; Preoperatif ilk değer, indüksiyon sonrası, entübasyon sonrası 5., 10., 15., dk. ile operasyon süresince her 15 dk.’da bir kaydedildi. Volatil ajan cerrahi işlem tamamen bittikten sonra kapatıldı. Anestezik ilaçların kapatılmasından ekstübasyona kadar geçen süre ekstübasyon zamanı, sesli uyaranla göz açmaya kadar geçen süre göz açma süresi olarak değerlendirildi.
