Angioedema ınduced by tramadol: a case report

Abstract
Tramadol has a safer profile compared to opioids due to its lower potential of respiratory depression and, therefore, is increasingly used for treatment of post- operative pain. This case report presents massive angioedema following trama- dol administration in a female patient who was operated due to arteriovenous malformation and subarachnoid hemorrhage. Our patient was transferred to the intensive care unit (ICU) after five hours of operation for neurologic follow up. Due to postoperative pain, 25 mg of tramadol was injected via an intravenous line and 30 minutes later, the patient started to have difficulty in speaking, swol- len tongue, tachypnea and shortness of breath. The tongue swelled massively within minutes and protruded 4-5 cm out of oral cavity. She was diagnosed as massive angioedema; and, 250 mg methyl prednisolone, 8 mg dexamethasone and 45.5 mg pheniramine was administered intravenously. Edema of the tongue subsided following anti-allergic therapy and disappeared 48 hours later. This case indicates that, although considered to be a safe opioid, tramadol may cause life threatening allergic reactions such as massive angioedema.

ramadol, opioidlere göre daha az solunum depresyonu yapması sebebiyle gü- venilir bir profile sahiptir ve bu nedenle postoperatif dönemde ağrı tedavisinde giderek daha sık kullanılmaktadır. Bu olguda, arteriovenöz malformasyon ve su- baraknoid kanama sebebiyle opere edilen kadın hastada tramadol uygulanmasısonrası gelişen masif anjioödem tablosu sunulmaktadır. Hastamız beş saat süren operasyon sonrası nörolojik takip amacıyla yoğun bakım ünitesine alındı. Pos- toperatif analjezi amacıyla intravenöz yoldan 25 mg tramadol uygulanmasından yaklaşık yarım saat sonra konuşma güçlüğü, dilde şişme, taşipne ve nefes darlığışikayetleri gözlendi. Hastanın dilinin ödemli olduğu ve büyüyerek ağzın dışına4-5 cm kadar taştığı izlendi. Gelişen tablonun anjioödem olduğu düşünülerekintravenöz yoldan 250 mg metil prednizolon, 8 mg deksametazon ve 45,5 mgpheniramine uygulandı. Antiallerjik tedaviyi takiben dildeki ödemin giderek ge- rilediği ve 48 saat sonra tamamen kaybolduğu görüldü. Bu olgu, güvenilir biropioid ajan olarak bilinen tramadolün de masif anjioödem gibi hayatı tehditedebilecek allerjik yan etkilere yol açabileceğini göstermektedir.
