A comparison of low and high flow desflurane anaesthesia in children

Abstract

Objective: Low-flow anaesthesia has become increasingly common by using modern anaesthesia devices. We aimed to compare anaesthesia with low flow and high fresh gas flows in children. Methods: Forty children in the ASA I-II groups, aged 5-15 years who were oper- ated for upper and lower extremities and operation times between two and four hours, were included in this study. During the operation, 4-6% desflurane, 2 L min-1 N2O and 2 L min-1 O2 were given to the low dose group throughout the first 10 minutes (n=20). Afterwards, 50% O2, 50% N2O, 4-6% desflurane were given as one MAC. In the high flow group (n=20) 4-6% desflurane, 2 L min-1 N2O and 2 L min-1 O2 were given throughout the operation. Inspiratory and ex- piratory concentrations of O2, CO2, N2O and desflurane, vital parameters, BIS and MAC were measured. During the anaesthesia period, extubation period, and 24 hours after surgery arterial blood gas samples were taken. Results: Desflurane amount was 235±97 mL in the high flow group, and 106±26 mL in the low flow group. Although there were significant differences between the two groups in terms of inspired and expired O2, CO2, desflurane concentrations, and MAC levels, they were within clinically acceptable ranges. There was clinically no difference between the two groups. Lactate levels were higher in the low flow group (T1-T8 periods). There were no differences be- tween the two groups in peroperative and recovery hemodynamics, arterial blood gas parameters, liver and kidney function, postoperative recovery period (extubation and orientation time). Conclusion: Our results show that low-flow anaesthesia, with the use of appro- priate techniques and devices, close monitoring of blood gases and lactate levels, can be applied safely in children.

Amaç: Düşük akımlı anestezi yöntemi birçok avantajları sebebiyle, modern anestezi cihazları ile güvenli bir şekilde yaygın olarak kullanılmaya başlanmıştır. Çalışmamızda çocuklarda dezfluran ile düşük ve yüksek akımlı anestezi yöntemlerinin karşılaştırılması amaçlanmıştır. Yöntemler: ASA I-II, 5-15 yaş, 2-4 saat sürecek alt ve üst ekstremite cerrahisi geçirecek 40 çocuk çalışmaya dahil edildi. Düşük akım grubuna (DAG, n=20); anestezi başlangıcında; 2 L dk-1 N2O, 2 L dk-1 O2 ve %4-6 dezfluran verildi. Sonrasında, %50 O2+%50 N2O+%4-6 dezfluran, 1 MAK olacak şekilde taze gaz akımı 1 L dk-1 şeklinde ayarlanarak verildi. Yüksek akım grubuna (YAG, n=20); başlangıçla birlikte 2L dk-1 N2O, 2L dk-1 O2 ve %4-6 dezfluran, girişim boyunca aynı konsantrasyonda verildi. İnspirasyon ve ekspirasyon gaz karışımındaki dezfluran, N2O, O2 ve CO2 konsantrasyon ölçümleri, vital bulgu takibi, BIS ve MAK ölçümleri yapıldı. Anestezi süresince, ekstübasyon döneminde ve postoperatif 24. saatte arter kan gazı değerlendirildi. Bulgular: Kullanılan ortalama dezfluran miktarı YAGda 235±97 mL; DAGda106±26 mL ölçüldü. İnspire ve ekspire edilen O2, N2O, dezfluran konsantrasyonları ile MAK ortalamaları arasında anlamlı fark olmasına rağmen, normal sınırlarda olduğu ve klinik olarak gruplar arasında fark olmadığı görüldü. DAGda laktat değerleri anlamlı olarak yüksek bulundu (T1-T8 dönemi). Gruplar arasında; peroperatif ve derlenme hemodinamisi, perioperatif ve postoperatif arter kan gazı parametreleri, karaciğer ve böbrek fonksiyonları, göz açma ve oryantasyon zamanı açısından fark saptanmadı.Sonuç: Çocuklarda da düşük akımlı anestezi yönteminin, yeterli donanıma sahip anestezi cihazı ve uygun monitörizasyon şartlarında, yakın kan gazı ve laktatdüzeyi takibi ile güvenle uygulanabileceği sonucuna varılmıştır.
