The cardioprotective effects of total ıntravenous anaesthesia and ınhalation anaesthesia during on-pump coronary artery bypass graft (CBAG) surgery

Abstract
Objective: The cardioprotective effects of total intravenous anaesthesia and inhalational anaesthesia were compared in patients with a history of myocardial infarction within the last six months undergoing on-pump coronary artery bypass graft surgery (CABG). Methods: Forty consecutive patients were divided into two groups in a prospective, randomized study. Infusions of fentanyl and propofol were used for induction and maintenance of anaesthesia in the total intravenous anaesthesia (TIVA) group (n=20). In the inhalation anaesthesia (SEVO) group (n=20), sevoflurane and infusion of fentanyl were used. Intravenous rocuronium was used in both groups and mean arterial pressure was kept above 50 mmHg during surgery. Serum creatinine phosphokinase MB raction (CK-MB), cardiac isoform of troponin I (cTnI), liver enzymes, and brain natriuretic peptide (BNP) values were collected before operaion, and 24 and 72 hours after operation. Perioperative risk factors and duration of intensive care unit stay were investigated. Results: There was no difference between groups in terms of baseline characteristics and serum CKMB levels before operation, or 24 and 72 hours after operation. The postoperative serum troponin levels at 24 and 72 hours after operation were lower in the SEVO group than in the TIVA group (p=0.017, p=0.003, respectively). Likewise, the postoperative serum BNP levels at 24 and 72 hours after operation were lower in the SEVO group than in the TIVA group (p=0.039, p=0.004, respectively). Perioperative risk factors did not differ between groups. Conclusion: In patients with a history of MI during the last six months prior to on-pump CABG surgery, sevoflurane anaesthesia provides better myocardial protection than total intravenous anaesthesia.

Amaç: Son altı ay içinde miyokard infarktüs (MI) öyküsü ile on-pump koroner arter baypas greftleme cerrahisi (KABG) uygulanan hastalarda total intravenöz anestezi ile inhalasyon anestezisinin kardiyoprotektif etkileri karşılaştırıldı. Yöntemler: Kırk ardışık hasta prospektif, randomize çalışmada iki gruba ayrıldı. Total intravenöz anestezi (TİVA) grubunda (n=20); anestezi indüksiyon ve idamesinde infüzyonla fentanil ve propofol uygulanırken, inhalasyon anestezisi (SEVO) grubunda (n=20); anestezi indüksiyon ve idamesinde sevofluran yanı sıra infüzyonla fentanil uygulandı. İki grupta da intravenöz rokuronyum uygulandı ve girişim boyunca ortalama arter basıncının 50 mmHgnın üstünde olması sağlandı. Ameliyat öncesi, ameliyat sonrası 24. ve 72. saatlerde serumda kreatinin fosfokinaz MB fraksiyonu (CK-MB), kardiyak izoform troponin I (cTnI), karaciğer enzimleri, beyin natriüretik peptid (BNP) değerleri toplandı. Perioperatif risk faktörleri, yoğun bakım ünitesinde kalış süresi incelendi. Bulgular: İki grubun temel özellikleri yanı sıra ameliyat öncesi, ameliyat sonrası 24. ve 72. saatlerdeki serum CKMB değerleri benzerdi. SEVO grubunun ameliyat sonrası 24. ve 72. saatlerdeki serum troponin değerleri, TİVA grubuna göre düşüktü (sırası ile; p=0,017, p=0,003). SEVO grubunda ameliyat sonrası 24. ve 72. saatlerdeki serum BNP değerleri TİVA grubuna göre daha düşüktü (sırası ile, p=0,039, p=0,004). Ameliyat sırasındaki risk faktörleri gruplar arasında farklı değildi. Sonuç: On-pump KABG operasyonu öncesi son altı ay içerisinde Mİ öyküsü olan hastalarda, sevofluran inhalasyon anestezisi, TİVAya göre miyokard korunmasında daha etkilidir.
