Abstract

LUNG INVOLVEMENT IN PROGN-RESSIVE SYSTEMIC SCLEROSIS: HRCT FINDINGS The purpose of this study is to asses the role of High Resoluotion Computed Tomography (HRCT) in the detection involvement and to evaluate the severity of changes of lung morphology confirmet by HRCT in progressive systemic sclerosi HRCT scans were obtained in 16 patients with progressive systemic sclerosis. This HRCT findings were correlat bronchoalveolar lavage (BAL) results. The most frequent finding was interlobular septal thickening which was demonstrate patients (81.2 %). Parenchymal and subpleural micronodules were seen in 12 and 11 patients respectively (75.0 % and while honeycombing was seen in 5 patients (31.2 %). Pulmonary abnormalities were mainly bilateral, with a pred distribution in the inferior, posterior and peripheral parts of the lungs. In addition oesophageal dilatation was seen in 6 (37.5 %). BAL results did not show, significant correlation with the severity of pulmonary findings. In conclusion, HRCT is a sensitive method in detecting lung involvement in progressive systemic sclerosis. Moreover, it is of choice for evaluating prenchymal destruction in predicting the severity of lung damage.

Özet

Bu çalışmanın amacı progressif sistemik sklerozda akciğer hasarının belirlenmesinde yüksek rezolüsyonlu bilgisayarlı tomc (YRBT) rolünü saptamak ve YRBT'de bulunan morfolojik akciğer değişikliklerinin şiddetini belirlemektedir. Sistemik sklerozlu 16 olguda YRBT kesitleri elde edilmiş ve YRBT bulguları bronkoalveolar iavaj (BAL) sonuçları ile korele edil En sık rastlanan bulgu 13 olguda (% 81.2) gösterilen interlobuler septal kalınlaşmalardır. Bal beteği görünümü 5 olguda ( görülürken, parankimal ve subplevral mikronodüler 12 ve 11 hastada (% 75.0 ve % 68.7) izlenmiştir. Pulmoner bulgular bilateral olup, ana dağılım akciğerin inferior, posterior ve periferindedir. Ek olarak 6 hastada (% 37.5) özofâgial dil görülmüştür. BAL sonuçları pulmoner bulguların şiddeti ile anlamlı korelasyon göstermemektedir. Sonuç olarak, YRBT, progressif sistemik sklerozun akciğer tutulumunun belirlenmesinde duyarlı bir yöntemdir. Bundar akciğer hasarının şiddetini belirleyen parankimal destrüksiyonun değerlendirilmesinde başvurulması gereken bir yöntemdir.
