The effect of clarithromycin with aminophylline treatment on serum theophylline concentration

Abstract

Clarithromycin, a new macrolide, is suggested not to affect the serum theophylline levels. There is not enough investigati-on about increasing effect ofaminophylline with darithromycin on serum theophylline levels. To determine the clinical re-levance of the interaction ofaminophylline with darithromycin, we measured serum theophylline levels of 30 patients who were hospitalized with the diagnosis of chronic obstructive pulmonary disease. Theophylline levels were measured at the end of third day in patients taking aminophylline therapy and then 500 mg/day darithromycin was added to the therapy in the fourth day for 5 days and again we measured serum theophylline levels. The serum theophylline levels before and after aminophylline with darithromycin therapy were 10.42±1.84ucg/mL and W.91±2.19 ucg/mL, respectively. These va-lues express an increase of 4.8% in serum theophylline levels after aminophylline with darithromycin therapy. In conclusion, we determined that the use ofaminophylline with darithromycin is a reliable combination in the treatment of acute exacerbations in patients with COPD.

Özet

Yeni bir makrolid olan klaritromisinin serum teofılin düzeyini etkilemediği savunulmaktadır. Aminofilinle birlikte kullanı-lan klaritromisinin serum teofılin düzeyini artırıp artırmadığına dair yeterli çalışma yoktur. Kronik obstrüktif akciğer has-talığı (KOAH) akut eksaserbasyonu tanısı ile hospitalize edilen 30 hastayı çalışmaya aldık. Bu hastalar son 10 gündür te-ofılin préparât! almamış kimselerdi. Üç gün boyunca aminofılin tedavisi aldıktan sonra 3. gün sonunda (saat 15.00) serum teofılin düzeylerine bakıldı. Tedavinin 4. gününde 5 gün süreli klaritromisin 500 mg/gün tedavisi eklendi. Tedavi sonunda ve aynı saatte tekrar serum teofılin düzeyleri ölçüldü. Klaritromisin+aminofilin tedavisi öncesi ve sonrası serum teofılin dü-zeyleri sırasıyla, 10.42±1.84 ucg/mLve 10.91±2.19 ucg/mLolarak saptandı. Bu değerler klaritromisin+aminofilin tedavisi sonrası serum teofılin düzeyinde olan %4.8'lik bir artışı ifade etmektedir. Sonuç olarak, KOAH'ın akut eksaserbasyonlarının tedavisinde klaritromisin+aminofilin kombinasyonunun oldukça gü-venli olduğu sonucuna vardık
