The efficacy of ifosfamide, vinblastine plus mitomycin for local advanced and metastatic non-small-lung cancer

Abstract

The efficacy, toxicity and feasibility of the combination of lfosfamide, Vinblastin, mitomycine-C were evaluated in 43 pati-ents with stage IUB and IV non-small cell lung cancer. Treatment consisted of ifosfamideß gr/m2/d with mesna) and vinb-lastine (6 mg/m2/d) and mitomycin-C (6 mg/m2/d) given intravenously on day 1 every 28 days. Eligibility criteria inclu-ded adequate hématologie, hepatic, arid renal functions; no prior chemotherapy; and an Eastern Cooperative Oncology Gro-up (ECOG) performance status (PS) of O to 2. All patients were male. There were 3.7% (1/27) complete response and 44.4% (12/27) partial response in stage UIB. There were 37.4% (6/16) partial response in stage IV. Median survival time and 1.5 -year survival were 330 days and 20% in stage HIB. Median survival time and 1-year survival were 251 days and 13% in sta-ge IV. Leukopenla was the most frequent toxicity: grades 3-4 neutropenia was observed in 12.3% of the cycles and 5 episo-des of febrile neutropenia were observed in 159 courses. Multiuariate analysis identified three significant predictive factors with Independent prognostic value: response to chemotherapy (p=0.012), stage (p=0.043), performance status (p=0.017). The combination of ifosfamide, vinblastine, mitomycin-C in the dose and schedule employed in this trial shows high res-ponse rate with acceptable toxicities.

Evre UlB ve IV küçük hücreli dışı akciğer kanserli 43 hastada ifosfamid, Vinblastin, mitomisin-C kombinasyonunu etkinli-ği, toksisitesi, uygulanabilirliği araştırıldı. Tedavi, her 28 günde, birinci günde uygulanan ifosfamid (3 gr/m2/gün, mesna 3 gr/m2)), Vinblastin (6mg/m2/gün), mitomisin-C (6 mg/m2/gün) içeriyordu. Dahil edilme kriterleri, yeterli hematolojik, hepatik, renal fonksiyonlara sahip olma, Eastern Cooperative Oncology Group (ECOG) performans durumu 0-2 olma ve daha önce kemoterapi almamış olmayı içeriyordu. Tüm hastalar erkekti. Eure IlIB'de, %3.7 (1/27) tam yanıt ve %44.4 (12/27) kısmi yanıt vardı. Evre IV'de, %37.4 (6/16) kısmi yanıt vardı. Evre IUB de ortalama yaşam süresi ve 1.5 yıllık ya-şam, 330 gün ve %20 idi. Evre IV'de, ortalama yaşam süresi ve 1-yıllık yaşam süresi, 251 gün ve %13 idi. Lökopeni en sık toksisiteydi; grade 3-4 nötropeni siklusların %12.3'de gözlendi. 159 siklusda, 5 febril nötropeni atağı gözlendi.
